Comparison on the Effects of Different Doses of Levothyroid Hormone in the Treatment of Elderly Patients with Subclinical Hypothyroidism

目的对比不同剂量左甲状腺激素治疗老年亚临床甲减患者的临床效果。方法选取2019年1月至2022年6月我院收治的100例老年亚临床甲减患者,随机分为低剂量组(初始使用剂量12.5μg/d)和高剂量组(初始使用剂量25μg/d)各50例。比较两组的治疗效果、甲状腺功能及不良反应。结果高剂量组治疗总有效率为94.00%,高于低剂量组的80.00%(P<0.05)。治疗后,高剂量组的TSH水平低于低剂量组,FT3、FT4水平高于低剂量组(P<0.05)。治疗期间,高剂量组不良反应发生率为24.00%,高于低剂量组的8.00%(P<0.05)。结论与低剂量左甲状腺激素相比,高剂量左甲状腺激素治疗老年亚临床甲减效果更佳,可显著促进患者甲状腺功能恢复,但不良反应发生率较高,临床应根据患者具体情况选择合适用药剂量。

Objective To compare the clinical effects of different doses of levothyroxine hormone in the treatment of elderly patients with subclinical hypothyroidism.Methods 100 elderly patients with subclinical hypothyroidism admitted to our hospital from January 2019 to June 2022 were randomly divided into low-dose group(initial dose 12.5μg/d)and high-dose group(initial dose 25μg/d),with 50 cases in each group.The treatment effect,thyroid function and adverse reactions of the two groups were compared.Results The total effective rate of treatment in the high-dose group was 94.00%,higher than 80.00% in the low-dose group(P<0.05).After treatment,the TSH level of the high-dose group was lower than that of the low-dose group,and the FT3 and FT4 levels were higher than those of the low-dose group(P<0.05).During the treatment,the incidence of adverse reactions in the high-dose group was 24.00%,higher than 8.00% in the low-dose group(P<0.05).Conclusions Compared with low-dose levothyroxine,high-dose levothyroxine has better effect in the treatment of elderly patients with subclinical hypothyroidism,and can significantly promote the recovery of thyroid function,with higher incidence of adverse reactions,so the clinic should choose the appropriate dose according to the specific condition of patients.
