延续干预联合家庭肠内营养支持对食管癌手术患者营养指标、免疫功能、希望水平及生活质量的影响

Effect of continuous intervention combined family enteral nutrition support on nutritional indexes,immune function,hope level and quality of life in patients undergoing esophageal cancer surgery

目的探讨延续干预联合家庭肠内营养支持对食管癌手术患者营养指标、免疫功能、希望水平及生活质量的影响。方法根据治疗方式的不同将80例接受手术治疗的食管癌患者分为家庭肠内营养组和联合干预组,每组40例,家庭肠内营养组患者给予家庭肠内营养支持,联合干预组患者给予延续干预联合家庭肠内营养支持。比较两组患者的营养指标[白蛋白(ALB)、前白蛋白(PA)、血红蛋白(Hb)]、免疫功能指标[免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)]、希望水平(Herth希望量表)、生活质量[生活质量综合评定问卷(GQOLI-74)]及满意度。结果干预后,两组患者的ALB、PA、Hb、IgA、IgG、IgM水平均高于本组干预前,联合干预组患者的ALB、PA、Hb、IgA、IgG、IgM水平均高于家庭肠内营养组,差异均有统计学意义(P<0.05)。干预后,两组患者采取积极的行动、与他人保持亲密的关系评分及总分均高于本组干预前,联合干预组患者采取积极的行动、与他人保持亲密的关系评分及总分均高于家庭肠内营养组,差异均有统计学意义(P<0.05)。干预后,两组患者物质生活、躯体功能、心理功能、社会功能评分均高于本组干预前,联合干预组患者物质生活、躯体功能、心理功能、社会功能评分均高于家庭肠内营养组,差异均有统计学意义(P<0.05)。联合干预组患者的总满意度明显高于家庭肠内营养组(P<0.01)。结论给予食管癌手术患者延续干预联合家庭肠内营养支持,能够改善患者的营养指标和免疫功能,提高患者的希望水平和生活质量。

Objective To analyze the effect of continuous intervention combined family enteral nutrition support on nutritional indexes,immune function,hope level and quality of life in patients undergoing esophageal cancer surgery.Method A total of 80 patients with esophageal cancer treated by surgery were divided into family enteral nutrition group(n=40,received family enteral nutrition support)and combined intervention group(n=40,received continuous intervention+family enteral nutrition support)according to the different intervention methods.The nutritional indexes[albumin(ALB),prealbumin(PA)and hemoglobin(Hb)],immune function indexes[immunoglobulin A(IgA),immunoglobulin G(IgG)and immunoglobulin M(IgM)],hope level(Herth hope index scale),quality of life[generic quality of life inventory-74(GQOLI-74)]and satisfaction were compared between the two groups.Result After intervention,the levels of ALB,PA,Hb,IgA,IgG and IgM in both groups were higher than those before intervention,with higher levels in the combined intervention group than those in the family enteral nutrition group(P<0.05).After intervention,the scores of taking positive action,maintaining close relationship with others and total scores were higher than those before intervention,with higher scores in the combined intervention group than those in the family enteral nutrition group(P<0.05).After intervention,the scores of material life,physical function,psychological function and social function in the combined intervention group were higher than those in the family enteral nutrition group(P<0.05).The total satisfaction rate of combined intervention group was higher than that of family enteral nutrition group(P<0.01).Conclusion Continuous intervention combined family enteral nutrition support for patients undergoing surgery for esophageal cancer can improve the nutritional indexes,immune function,hope level and quality of life of patients.
