影响慢性心力衰竭患者预后的相关危险因素分析

Analysis of related risk factors affecting the prognosis of patients with chronic heart failure

目的 探讨影响慢性心力衰竭患者预后的相关危险因素。方法 回顾性分析2019年5月一2020年6月浙江省医疗健康集团杭州医院收治的慢性心力衰竭患者100例,收集患者的相关资料,包括美国纽约心脏病协会(NYNA)心功能分级情况的心力衰竭患者的血清Galectin-3、PTX-3、左心室质量指数(LVMI)、左心室射血分数(LVEF)、B型钠尿肽(BNP)水平,随访1年,根据患者有无发生主要心血管不良事件分为预后不良组与预后良好组,采用单因素及多因素Logistic回归分析影响患者预后的独立影响因素。结果 单因素分析结果显示,心功能分级、Galectin-3、PTX-3、BNP、LVMI、LVEF水平是影响慢性心力衰竭患者的因素,差异有统计学意义(P<0.05);多因素Logistic回归分析显示,心功能分级、Galectin-3、PTX-3、BNP、LVEF均为影响慢性心力衰竭患者预后的独立影响因素(P<0.05)。结论 Galectin-3、PTX-3、BNP、LVEF是影响慢性心力衰竭患者预后的独立影响因素。

Objective To explore the relevant risk factors affecting the prognosis of patients with chronic heart failure.Methods A retrospective analysis was made of 100 patients with chronic heart failure who were admitted to Hangzhou Hospital of Zhejiang Medical and Health Group from May 2019 to June 2020,and the relevant data of the pa­tients were collected,including the New York Heart Association(NYNA)cardiac function classification Serum Galectin-3,PTX-3,left ventricular mass index(LVMI),left ventricular ejection fraction(LVEF),and B-type natriuretic peptide(BNP)levels in heart failure patients were followed up for 1 year.The occurrence of major cardiovascular adverse events was di­vided into poor prognosis group and good prognosis group.Univariate and multivariate Logistic regression analysis was used to analyze the independent factors affecting the prognosis of patients.Results Univariate analysis showed that cardiac func­tion grade,Galectin-3,PTX-3,BNP,LVMI,and LVEF levels were factors affecting patients with chronic heart failure,and the differences were statistically significant(P<0.05);multivariate results showed that,Cardiac function class,Galectin-3,PTX-3,BNP,and LVEF were all independent factors affecting the prognosis of patients with chronic heart failure(P<0.05).Conclusion Galectin-3,PTX-3,BNP,and LVEF are independent factors affecting the prognosis of patients with chronic heart failure.
