棱术盆炎汤治疗湿热蕴结型慢性盆腔炎伴盆腔积液的临床研究

Clinical Study of Lengzhu Penyan Decoction in the Treatment of Chronic Pelvic Inflammatory Disease with Pelvic Effusion of Dampness-Heat Accumulation Type

目的探讨棱术盆炎汤治疗湿热蕴结型慢性盆腔炎(Chronic pelvic inflammatory disease,,CPID)伴盆腔积液患者的临床疗效。方法选取2019年1月—2021年1月期间北京市昌平区中西医结合医院收治的79例CPID伴盆腔积液患者,采用随机数字表法分为对照组41例和治疗组38例。对照组采用妇炎舒胶囊治疗,治疗组在对照组基础上加用棱术盆炎汤灌肠治疗,连续治疗14 d,并随访1个月。于随访结束当天,评估两组患者疗效,并检测两组阴道清洁度情况;分别于治疗前、治疗14 d后,评估并比较两组患者腹痛情况、盆腔积液体积及中医证候积分。结果治疗后治疗组总有效率94.74%(36/38)高于对照组78.05%(32/41),差异有统计学意义(P<0.05)。治疗后两组患者阴道清洁度均较治疗前明显改善,差异有统计学意义(P<0.05);且治疗组优于对照组,差异有统计学意义(P<0.05)。治疗后两组患者腹痛视觉模拟评分均较治疗前明显降低,差异有统计学意义(P<0.05);且治疗组优于对照组,差异有统计学意义(P<0.05)。治疗后两组患者盆腔积液体积、盆腔炎性包块均较治疗前明显缩小,差异有统计学意义(P<0.05);且治疗组盆腔积液体积、盆腔炎性包块均较对照组明显缩小,差异有统计学意义(P<0.05)。治疗后两组患者中医证候积分均较治疗前明显降低,差异有统计学意义(P<0.05);且治疗组中医证候积分较对照组明显降低,差异有统计学意义(P<0.05)。结论棱术盆炎汤可有效提高湿热蕴结型CPID伴盆腔积液患者的治疗效果,更利于减轻疼痛程度,降低中医证候积分,改善阴道清洁度和盆腔积液情况。

Objective To analyze the clinical efficacy of Lengzhu Penyan Decoction in the treatment of chronic pelvic inflammatory disease(CPID)patients with pelvic effusion of dampness-heat accumulation type.Methods A total of 79 CPID patients with pelvic effusion treated in Beijing Changping Hospital of Integrated Chinese and Western Medicine from January 2019 to January 2021 were included.They were divided into a control group(41 cases)and a treatment group(38 cases)by random number table method.The control group was treated with Fuyanshu Capsule,while the treatment group was given Lengzhu Penyan Decoction by enema on the basis of the control group.The patients were treated for 14 consecutive days and followed up for 1 month.On the day of the end of follow-up,the efficacy of the two groups was evaluated and the vaginal cleanliness was detected.The abdominal pain,pelvic effusion volume and traditional Chinese medicine(TCM)syndrome score of the two groups before treatment and 14 days after treatment were evaluated and compared.Results The total effective rate of the treatment group was 94.74%(36/38)higher than 78.05%(32/41)of the control group(P<0.05).After treatment,the vaginal cleanliness of the two groups were improved(P<0.05),and the improvement was more significant in the treatment group than the control group(P<0.05).Additionally,the visual analogue scale(VAS)score and TCM syndrome score in the two groups after treatment were lowered,and the pelvic effusion volume and pelvic inflammatory mass were reduced compared with those before treatment(all P<0.05),with the treatment group having more decreases than the control group(all P<0.05).Conclusion Lengzhu Penyan Decoction effectively enhances the therapeutic effect of CPID patients with pelvic effusion of dampness-heat accumulation type.It is conducive to reducing the degree of pain and TCM syndrome score and improving vaginal cleanliness and pelvic effusion.
