可摘局部义齿在重度老年牙周炎伴牙列缺损患者中的应用及对牙周状况的效果改善

Effects of removable partial denture on elderly patients with severe periodontitis with dentition defects and improvement of periodontal condition

目的可摘局部义齿在重度老年牙周炎伴牙列缺损患者中的应用及对牙周状况的效果改善探究。方法选取2021年1月至2022年1月来内蒙古通辽市医院和内蒙古赤峰学院附属医院接受治疗的重度老年牙周炎伴牙列缺损患者62例,采用随机数表法分为观察组和对照组,每组各31例,对照组患者进行常规修复治疗,观察组进行可摘局部义齿修复治疗。比较两组患者的修复效果、舒适感和咀嚼功能评分、牙周状况、生活质量评分、满意度。结果观察组患者的治疗修复总优良率高于对照组,差异有统计学意义(P<0.05)。观察组患者的舒适感和咀嚼功能评分均高于对照组,差异有统计学意义(P<0.05)。治疗前,两组患者的牙龈指数、出血指数和龈沟出血指数比较,差异无统计学意义(P>0.05),治疗后均有好转且观察组患者的评分低于对照组,差异有统计学意义(P<0.05)。治疗前两组患者的生活质量评分比较,差异无统计学意义(P>0.05),治疗后均有一定改善,并且观察组的分值高于对照组,差异有统计学意义(P<0.05)。相较于对照组,观察组患者对修复治疗的总满意度高于对照组,差异有统计学意义(P<0.05)。结论可摘局部义齿在重度老年牙周炎伴牙列缺损患者中的应用具有较好的临床治疗效果,可有效改善患者的牙周状况,提高生活质量,实用价值显著,可推广使用。

Objective To investigate the effects of removable partial denture on elderly patients with severe periodontitis with dentition defects and improvement of periodontal condition.Methods A total of 62 elderly patients with severe periodontitis with dentition defects who were treated in Tongliao City Hospital of Inner Mongolia Autonomous Region and the Affiliated Hospital of Chifeng University in Inner Mongolia Autonomous Region from January 2021 to January 2022 were selected and divided into an observation group(n=31)and a control group(n=31)according to the randomiz ed number table method.The control group received conventional repair treatment,while the observation group received repair treatment with removable partial denture.The effect of repair treatment,comfort level and masticatory function scores,periodontal status,quality of life(QoL)scores,and satisfaction of patients in the two groups were compared and analyzed.Results The overall excellent rate of repair treatment in the observation group was higher than that in the control group,with statistically significant difference(P<0.05).The comfort level and masticatory function scores in t he observation group were higher than those in the control group,with statistically significant differences(P<0.05).Before the treatment,there were no statistically significant differences between the two groups in the gingival index,bleeding index and gingival sulcus bleeding index(P>0.05),while after treatment,these indexes in the two groups improved and those in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).Before treatment,there was no statistically significant difference between the two groups in the QoL scores(P>0.05),while after treatment,the scores in the two groups improved and those in the observation group were higher than those in the control group,with statistically significant difference(P<0.05).The overall satisfaction of the patients in the observation group was higher than that in the control group,with statistically significant difference(P<0.05).Conclusion The removable partial denture is effective in improving the periodontal condition and upgrading the QoL of elderly patients with severe periodontitis with dentition defects,with significant practical value,and is worthy of promotion.
