摘要
传统超声心动图主要是通过左心室射血分数(LVEF)来反映左心室功能,但其作为一个整体功能参数,不能体现局部心肌功能,且因受到后负荷的影响存在不准确性。二维斑点追踪成像(2D-STI)通过追踪各时间点的心肌运动回声获得心肌应变率,从而评估心肌整体和局部功能,但是该技术仍存在负荷依赖的局限性。无创左心室压力应变环(PSL)通过结合2D-STI和后负荷,将心肌应变和心室压力进行了联合,在评价心肌功能时消除了后负荷的影响,能更加准确地对整体和区域心肌功能进行定量分析,是近年来研究的热点。本文主要对无创左心室PSL评估心肌功能的研究进展进行综述。
Conventionally,the left ventricular function is mainly reflected by echocardiography through left ventricular ejection fraction(LVEF).However,as a whole function parameter,it can’t reflect regional myocardial function,and there is inaccuracy due to the impact of afterload.Two-dimensional speckle tracking imaging(2D-STI)can obtain myocardial strain rate by tracking the echo of myocardial motion at each time point,so as to evaluate the overall and local function of myocardium.However,this technique still has the limitation of load dependence.The non-invasive left ventricular pressure strain loop(PSL)combines the myocardial strain and ventricular pressure by combining 2D-STI with afterload,which eliminates the impact of afterload in the evaluation of myocardial function and can conduct the quantitative analysis of the overall and regional myocardial function more accurately.It is a research hotspot in recent years.The research progress of non-invasive left ventricular PSL in the evaluation of myocardi al function is mainly reviewed in this paper.
