血清淀粉样蛋白A、血常规、C反应蛋白在儿科感染性疾病中的应用价值研究 被引量:1

Study on the application values of serum amyloid A,blood routine and C-reactive protein in pediatric infectious diseases

摘要 目的探讨血清淀粉样蛋白A(SAA)、血常规、C反应蛋白(CRP)在儿科感染性疾病中的应用价值。方法选取贺州市人民医院2020年11月至2022年7月收治的78例病毒感染患儿作为病毒组(n=78),62例细菌感染患儿作为细菌组(n=62),并选取67例非感染性疾病患儿作为对照组(n=67),三组患儿均接受血清检查。比较三组患儿SAA、血常规白细胞计数、CRP水平,并评价SAA、血常规白细胞计数、CRP诊断儿科感染性疾病的效能。结果细菌组和病毒组患儿血清SAA、白细胞计数、CRP水平高于对照组患儿(P<0.05);细菌组患儿血清SAA、白细胞计数、CRP水平高于病毒组患儿(P<0.05);ROC曲线结果提示,SAA诊断细菌感染的AUC值为0.834,敏感度为80.6%,特异度为85.1%;白细胞计数诊断细菌感染的AUC值为0.782,敏感度为79.0%,特异度为73.1%;CRP诊断细菌感染的AUC值为0.879,敏感度为88.7%,特异度为92.5%;ROC曲线结果提示,SAA诊断病毒感染的AUC值为0.841,敏感度为85.9%,特异度为82.1%;白细胞计数诊断病毒感染的AUC值为0.783,敏感度为74.4%,特异度为71.6%;CRP诊断病毒感染的AUC值为0.817,敏感度为69.2%,特异度为79.1%。结论SAA、血常规、CRP在儿科感染性疾病中的诊断值较高,其中SAA对于病毒感染最敏感,而细菌感染方面,CRP最敏感。 Objective To investigate the application values of serum amyloid A(SAA),blood routine and C-reactive protein(CRP)in pediatric infectious diseases.Methods A total of 78 children with viral infections admitted to Hezhou People’s Hospital from November 2020 to July 2022 were selected as the viral group(n=78),62 children with bacterial infections as the bacterial group(n=62),and another 67 children with non-infectious diseases as the control group(n=67).All the three groups of children underwent serum examination.The levels of SAA,CRP and blood routine(white blood cell count)were compared among the three groups of children,and the efficacy of SAA,blood routine(white blood cell count),and CRP in diagnosing pediatric infectious diseases was evaluated by confirmed diagnosis.Results The levels of serum SAA,white blood cell count,and CRP in the bacterial and viral groups were higher than those in the control group(P<0.05),and those in the bacterial group were higher than those in the viral group(P<0.05).The receiver operating characteristic(ROC)curve suggested that the area under the curve(AUC)was 0.834,the sensitivity was 80.6%,and the specificity was 85.1%for SAA in diagnosing bacterial infections;the AUC was 0.782,the sensitivity was 79.0%,and the specificity was 73.1%for white blood cell count in diagnosing bacterial infections;and the AUC was 0.879,the sensitivity was 88.7%,and the specificity was 92.5%for CRP in diagnosing bacterial infections.The ROC curve also suggested that the AUC was 0.841,the sensitivity was 85.9%,and the specificity was 82.1%for SAA in diagnosing viral infections;the AUC was 0.783,the sensitivity was 74.4%,and the specificity was 71.6%for white blood cell count in diagnosing viral infections;and the AUC was 0.817,the sensitivity was 69.2%,and the specificity was 79.1%for CRP in diagnosing viral infections.Conclusion The SAA,blood routine,and CRP are all of high value in diagnosing pediatric infectious diseases,with SAA being the most sensitive for viral infections and CRP for bacterial infections.
