超声引导下臂丛神经阻滞、关节腔穿刺在肩关节疼痛治疗中的应用效果评价

Application Effect Evaluation of Ultrasound Guided Brachial Plexus BlockJoint Cavity Puncture in Treatment of Shoulder Pains

目的 探究超声引导下臂丛神经阻滞、关节腔穿刺在肩关节疼痛治疗中的应用效果。方法 选取我院于2017年8月-2021年3月收治的60例肩关节疼痛患者作为研究对象,分组方式为电脑随机分组,对照组采取关节腔穿刺,观察组采取超声引导下臂丛神经阻滞,将两组的疼痛评分、术后30min膈肌麻痹程度、术后不同时间前臂MBS评分、治疗效果进行对比。结果 观察组治疗后1d、治疗后3d、治疗后1周疼痛评分分别为(3.01±1.02)分、(2.02±1.42)分、(1.02±0.45)分,均低于对照组(P<0.05)。观察组肩关节疼痛患者术后30min膈肌麻痹程度相较于对照组更具有优势(P<0.05)。术后4h对比两组前臂MBS评分可知,观察组显著大于对照组(P<0.05),术后12h两组前臂MBS评分无显著的统计学差异(P>0.05);观察组治疗总有效率高于对照组(P<0.05)。结论 超声引导下臂丛神经阻滞在肩关节疼痛治疗中具有显著效果,能够使患者机体疼痛得以减轻,且对患者的膈肌麻痹程度及前臂屈肘、屈腕、屈指影响更小,值得研究和推广。

Objective To explore effect of ultrasound-guided brachial plexus block and joint cavity puncture in treatment of shoulder pains.Methods The paper chose 60 patients with shoulder joint pains treated in our hospital from August 2017 to March 2021 as main study objects by computer random grouping.Control group was treated with joint cavity puncture,and observation group with ultrasound guided brachial plexus block.Pain scores,degree of diaphragm paralysis 30 minutes after operation,forearm MBS scores at different times after operation,and treatment effect were compared between two groups.Results Pain scores of observation group were(3.01±1.02)points,(2.02±1.42)points and(1.02±0.45)points respectively on the first day,the third day and the first week after treatment,lower than control group(P<0.05).Degree of diaphragmatic paralysis in patients with shoulder joint pain in observation group 30 minutes after operation was more superior than control group(P<0.05).Forearm MBS scores between two groups 4 hours after operation of observation group were significantly higher than control group(P<0.05),there was no significant difference in forearm MBS scores between two groups 12 hours after operation(P>0.05).Total effective rate of treatment in observation group was higher than control group(P<0.05).Conclusion Ultrasound guided brachial plexus block can achieve significant effect in treatment of shoulder joint pains,can reduce body pains with less impact on degree of diaphragm paralysis and forearm flexion of elbow,wrist,and finger,which is worthy of research and promotion.
