糖皮质激素联合泛昔洛韦治疗带状疱疹的疗效观察

Curative Effect Observation of Glucocorticoid Combined with Famciclovir inTreatment of Herpes Zoster

目的 分析带状疱疹采用泛昔洛韦联合小剂量糖皮质激素进行治疗的效果和安全性。方法 选择本院2018年3月-2020年5月诊治的带状疱疹患者126例,按照随机数表法平均分为对照组和研究组。所有患者均口服昔洛韦、吲哚美辛,并肌注维生素B1、B12,重组人干扰素α-2b凝胶外用,连续治疗10d。研究组患者增加口服小剂量泼尼松。比较两组治疗效果和安全性。结果 研究组在开始止痛、止疱、结痂、皮损痊愈等方面用时都显著缩短(P<0.05);研究组比对照组在疼痛缓解积分和发生后遗神经痛方面都有一定程度的降低(P<0.05),两组治疗10d时疼痛缓解积分差异无统计意义(P>0.05);治疗后两组IL-2、IL-10水平均有不同程度改善,研究组各项值改善幅度更明显(P<0.05);两组均未出现严重不良反应。结论 带状疱疹常规治疗早期增加糖皮质激素治疗效果明显。

Objective To analyze effect and safety of famciclovir combined with low-dose glucocorticoid in treatment of herpes zoster.Methods The paper chose 126 patients with herpes zoster diagnosed and treated in our hospital from March 2018 to May 2020,and divided them into control group and study group randomly.All cases took orally ciclovir,indomethacin,and intramuscular injection of vitamins B1,B12,recombinant human interferonα-2b gel was applied externally for 10 days.Study group was treated with small doses of prednisone orally.Effect and safety between two groups was compared.Results In study group,time for pain relief,blister arrest,scab formation,and skin lesion recovery was significantly shorter(P<0.05);scores of pain relief and occurrence of posttraumatic neuralgia in study group was lower than control group to some certain extent(P<0.05).After treatment,levels of IL-2 and IL-10 in two groups were improved to varying degrees,improvement of each value in study group was more significant(P<0.05),there were no serious adverse reactions in both groups.Conclusion Early glucocorticoid treatment can achieve obvious effect for herpes zoster.
