小切口开窗式髓核摘除术治疗腰椎间盘突出症患者的临床效果分析

目的 分析对腰椎间盘突出症患者实施小切口开窗式髓核摘除术治疗的效果。方法 抽取2019年1月-2021年10月本院收治的腰椎间盘突出症患者30例作为本文的观察对象,并根据随机数字法将其分成两组,分别接受不同的治疗方法,参照组接受传统手术治疗,研究组接受小切口开窗式髓核摘除术治疗,对比两组的治疗有效率、手术相关指标、炎症因子水平、疼痛评分以及应激指标。结果 对比治疗有效率,研究组高于对照组,两组统计学意义显著(P<0.05)。对比手术相关指标,研究组的手术用时以及下床活动时间较短,手术切口较短,出血量较少,与参照组相比具有统计学差异(P<0.05)。对比炎症因子水平,治疗前对比差异无统计学意义(P>0.05);治疗后,研究组的超敏C反应蛋白(hs-CRP)以及降钙素原(PCT)均低于参照组,差异具有统计学意义(P<0.05)。对比VAS评分,治疗前结果无统计学意义(P>0.05);治疗后差异有统计学意义(P<0.05)。观察组的腰部、腿部VAS评分均较低。对比应激指标,治疗前无统计学意义(P>0.05),治疗后,研究组的平均动脉压(MAP)以及心率(HR)指标均低于参照组(P<0.05)。结论 对腰椎间盘突出症患者实施小切口开窗式髓核摘除术治疗的价值较高,可有效缩短手术用时与下床活动时间,且手术切口较小,出血量较少,同时还可有降低疼痛,可推广。

Objective To analyze effect of small incision fenestration of nucleus pulposus for patients with lumbar disc herniation.Methods The paper chose 30 patients with lumbar disc herniation admitted to our hospital from January 2019 to October 2021 as study subjects,and divided them into two groups according to random number method.They received different treatment methods.Reference group was treated with traditional surgical treatment,and study group with small incision fenestration nucleus pulposectomy.Treatment effect,surgical related indicators,inflammatory factor levels,pain scores and stress indicators were compared between two groups.Results Effective rate of study group was significantly higher than control group(P<0.05);for operation related indicators,study group had shorter operation time and out of bed time,shorter surgical incision and less bleeding,which was statistically different from control group(P<0.05);for inflammatory factors level,there was no statistically significant difference before treatment(P>0.05).After treatment,levels of hypersensitive C-reactive protein(hs-CRP)and procalcitonin(PCT)in study group were significantly lower than control group(P<0.05);VAS scores before treatment showed no statistical significance,(P>0.05),difference after treatment was statistically significant(P<0.05).VAS scores of waist and leg in observation group were lower;stress indicators showed no statistical significance before treatment(P>0.05).After treatment,mean arterial pressure(MAP)and heart rate(HR)of study group were lower than control group(P<0.05).Conclusion Small incision fenestration of nucleus pulposus has high value in treatment of lumbar disc herniation,can shorten operation time and out of bed time effectively,with less operation incision,bleeding amount and pains,which can be promoted.
