四维牵引联合针刺整脊手法治疗慢性非特异性下腰痛临床研究

Clinical study of four-dimensional traction combined with acupuncture chiropractic manipulation in the treatment of chronic non-specific low back pain

目的:观察四维牵引联合针刺整脊手法对慢性非特异性下腰痛(CNLBP)患者腰背部疼痛的改善以及对腰椎-骨盆参数的影响。方法:选取60例CNLBP患者随机分为治疗组和对照组,每组30例。治疗组采用四维牵引联合针刺整脊手法治疗,对照组采用针刺联合整脊手法进行治疗,每组治疗14 d,比较两组疗效和治疗前后疼痛视觉模拟评分(VAS)、日本骨科协会评估治疗分数(JOA)、腰椎冠状面Cobb角、腰椎弓顶距离、腰椎前突指数、骶骨倾斜角(SS)、骨盆倾斜角(PI)、骨盆入射角(PT)、脊柱矢状轴(SVA)、骶-股距(SFD)。结果:观察组VAS评分、Cobb角、PI低于对照组,JOA评分、腰椎弓顶距离高于对照组,差异有统计学意义(P<0.05)。治疗后,两组腰椎前突指数、SS、PT、SVA、SFD比较差异均无统计学意义(P>0.05)。结论:四维牵引联合针刺整脊手法能够有效缓解CNLBP疼痛,纠正患者腰椎曲度异常,调节腰椎-骨盆参数。

Objective:To observe the effects of four-dimensional traction combined with acupuncture and chiropractic manipulation on patients with CNLBP.Methods:60 patients of CNLBP were randomly divided into treatment group and control group,30 cases in each group.The treatment group was treated with four-dimensional traction combined with acupuncture and chiropractic manipulation,while the control group was treated with acupuncture and chiropractic manipulation.Each group was treated for 14 days.The effects of the two groups were compared VAS,JOA,the coronal Cobb angle of the lumbar spine,the distance between the top of the lumbar curve,the anterior protrusion index of the lumbar spine SS,PI,PT,SVA and SFD before and after treatment.Results:VAS score,Cobb angle and PI of the observation group were lower than those of the control group,while JOA score and the distance between the lumbar curve and the top of the arch were higher than those of the control group,with statistical significance(P<0.05).After treatment,there were no significant differences in lumbar protrusion index,SS,PT,SVA and SFD between the two groups(P>0.05).Conclusion:Four-dimensional traction combined with acupuncture and chiropractic manipulation can effectively relieve the pain of CNLBP,correct the abnormal lumbar curvature and adjust the parameters of lumbar spine and pelvis.
