Professor LIU Baohou’s experience in treating allergic purpura nephritis

过敏性紫癜性肾炎为过敏性紫癜累及肾脏时出现的一系列临床表现的疾病,西医对本病无特异性治疗手段,短期效果尚可,停药减药后容易复发。针对这一情况,中医药在治疗此病时发挥了独特优势,在减轻激素带来的不良反应的同时可缓解临床症状、缩短病程、降低复发率。刘宝厚教授结合工作经验,提出的慢性肾炎中医辨证分型方案得到全国认可,已被广泛推广应用,在治疗过敏性紫癜性肾炎方面有自己独到的见解。刘宝厚教授认为过敏性紫癜性肾炎为本虚标实证,以肾络亏虚以及感受风、湿、热、毒、瘀为其基本病机,在整个病情发展过程中,热、虚、瘀三者既可单独存在又可兼夹为病。刘教授主张分期论治该病,急性期以清热解毒、活血通络为主,迁延期以益气养阴、佐以化瘀为主,总体将清热解毒、祛风通络、活血化瘀作为治疗紫癜性肾炎的基本治法。

Allergic purpura nephritis is a series of clinical manifestations of Henoch-Schönlein purpura when it affects the kidneys,Western medicine has no specific treatment for this disease,the short-term effect is acceptable,and it is easy to recur after stopping the drug and reducing the drug.In view of this situation,TCM medicine has played a unique advantage in the treatment of this disease,which can alleviate clinical symptoms,shorten the course of the disease and reduce the recurrence rate while reducing the adverse reactions caused by hormones.Based on his work experience,Professor LIU Baohou proposed a TCM differentiation and classification scheme for chronic nephritis,which has been widely promoted and applied,and has his own unique insights in the treatment of allergic purpura nephritis.Professor LIU Baohou believes that the main syndrome of allergic purpura nephritis is insufficient foundation and excessive appearance.Its basic pathogenesis is the deficiency of kidney and collaterals,and the invasion of the body by external pathogenic factors such as wind,dampness,heat,poison and blood stasis.During the development of the disease,heat,deficiency and blood stasis can cause the disease either individually or together.Professor LIU advocates treating the disease by stages.During the acute stage,the method of clearing heat and detoxifying,and promoting blood circulation and dredging collaterals is mainly used clinically.During the stable stage,the treatment method is mainly to nourish Qi(气)and Yin(阴),supplemented by removing blood stasis.The basic therapy for treatment of purpuric nephritis is the method of clearing heat and detoxifying,expelling wind and dredging collaterals,and promoting blood circulation and removing blood stasis.
