TAT、TM、PIC和t-PAIC在冠脉支架植入术后患者血清中的表达及与预后的关系

摘要 目的分析凝血酶抗凝酶复合物(TAT)、血栓调节蛋白(TM)、纤溶酶-抗纤溶酶复合物(PIC)、组织型纤溶酶原激活抑制复合物(t-PAIC)在冠脉支架植入术后患者血清中的表达及与预后的关系。方法选择2020年7月至2022年3月行冠脉支架植入术治疗患者73例,纳入观察组。按照1∶1原则在同期选择体检健康的受试者73例,纳入对照组。检测2组患者血清TAT、TM、PIC和t-PAIC水平,随访6个月统计不良心血管事件(MACE)发生情况,作为判断预后的指标。比较对照组和观察组血清TAT、TM、PIC和t-PAIC水平;分析非MACE组及MACE组血清TAT、TM、PIC和t-PAIC水平差异;探讨血清TAT、TM、PIC和t-PAIC水平与预后关系。结果观察组血清TM、PIC和t-PAIC水平均高于对照组(P<0.05),血清TAT水平低于对照组(P<0.05)。非MACE组及MACE组患者血清TAT水平差异无统计学意义(P>0.05)。MACE组患者血清TM、PIC和t-PAIC水平均高于非MACE组(均P<0.05)。血清TAT水平与预后(MACE)发生无关(P>0.05),血清TM、PIC和t-PAIC水平与预后(MACE)发生呈正相关(P<0.05)。结论冠脉支架植入术后患者血清TAT、TM、PIC和t-PAIC异常表达,其中血清M、PIC和t-PAIC水平与预后MACE发生有关,建议密切关注。 Objective To analyze the relationship between the serum expression of thrombin-antithrombin complex(TAT),thrombomodulin(TM),plasmin-alpha-2-antiplasmin complex(PIC),and tissue plasminogen activator-inhibitor complexes(T-PAIC)with prognosis in patients receiving coronary stent implantation.Methods Matching(1∶1)was performed for the patients receiving coronary stent implantation(observation group,n=73)and the subjects receiving physical examinations(control group,n=73)from July 2020 to March 2022.The serum levels of TAT,TM,PIC and T-PAIC were detected in the two groups.The incidence of major adverse cardiovascular events(MACE),defined as a prognostic indicator,was followed up for 6 months.The serum levels TAT,TM,PIC and T-PAIC were compared between observation group and control group,and between MACE group and non-MACE group.The relationship between serum levels of TAT,TM,PIC and T-PAIC with prognosis was assessed.Results The serum levels of TM,PIC and T-PAIC in the observation group were significantly higher than those in the control group(all P<0.05),and the serum TAT level was significantly lower than that in the control group(P<0.05),but no difference was significant in TAT level between the non-MACE group and the MACE group(P>0.05).Serum levels of TM,PIC and T-PAIC in MACE group were significantly higher than those in non-MACE group(all P<0.05).Serum TAT level did not correlate with the prognosis and predictors of MACE(P>0.05).In contrast,serum levels of TM,PIC and T-PAIC were positively correlated with prognosis and predictors of MACE(P<0.05).Conclusion Serum TAT,TM,PIC and T-PAIC are abnormally expressed after coronary stent implantation.Serum levels of TM,PIC and T-PAIC were correlated to prognosis and predictors of MACE,which should be closely monitored.
