多囊卵巢综合征合并非酒精性脂肪性肝病危险因素分析

Risk Factors for Patients with Polycystic Ovary Syndrome Complicated with Non-Alcoholic Fatty Liver Disease

目的:探讨多囊卵巢综合征(PCOS)合并非酒精性脂肪性肝病(NAFLD)的危险因素。方法:收集2020年5月-2020年12月因PCOS或NAFLD入住我院的308例女性患者临床资料,分为单纯PCOS组(122例)、PCOS合并NAFLD组(105例)和单纯NAFLD组(81例),另选择健康女性60例为对照组。测量身高、体重、腰围和臀围。抽取静脉血检测卵泡刺激素(FSH)、黄体生成素(LH)、睾酮(T)、谷丙转氨酶(ALT)、谷草转氨酶(AST)和谷氨酰转肽酶(GGT)、血清甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)和空腹血糖(FBG)。结果:与单纯PCOS组相比,PCOS合并NAFLD组BMI、腰臀比(WHR)、TC、TG、ALT和FBG更高;与单纯NAFLD组相比,PCOS合并NAFLD组LH、LH/FSH和T增加,差异均有统计学意义(均P<0.05)。Logistic回归显示,BMI、WHR、TC和TG水平越高,PCOS患者发生NAFLD的风险越高(均P<0.05)。结论:BMI、WHR、TC和TG是PCOS并发NAFLD的独立危险因素。

Objective:To investigate the risk factors of polycystic ovary syndrome(PCOS)complicated with non-alcoholic fatty liver disease(NAFLD).Methods:A total of 308 patients with PCOS or NAFLD from May 2020 to December 2020 were divided into the PCOS alone group(n=122),the PCOS combined with NAFLD group(n=105)and the NAFLD alone group(n=81).Another 60 healthy women were selected as the control group.Height,weight,waist circumference and hip circumference were measured.Venous blood samples were collected to detect follicle stimulating hormone(FSH),luteinizing hormone(LH),testosterone(T),alanine aminotransferase(ALT),aspartate aminotransferase(AST)and glutamyl transpeptidase(GGT),serum triglyceride(TG),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C)and fasting blood glucose(FBG).Results:Compared with the PCOS alone group,BMI,waist-to-hip ratio(WHR),TC,TG,ALT and FBG were higher in the PCOS combined with NAFLD group(all P<0.05).Compared to the NAFLD alone group,LH,LH/FSH and T in the PCOS combined NAFLD group were increased.Logistic regression analysis suggested that the higher levels of BMI,WHR,TC and TG increased the risk of NAFLD in PCOS patients(all P<0.05).Conclusion:BMI,WHR,TC and TG were independent risk factors for PCOS complicated with NAFLD.
