疟疾患者外周血T淋巴细胞亚群检测及临床意义

摘要 目的探讨疟疾患者外周血T淋巴细胞亚群的变化及其临床意义。方法选取2020年1月~10月期间广州医科大学附属市八医院就诊的49例病人标本,分为疟疾患者组(n=36)和健康对照组(n=13),采用流式细胞技术检测其外周血T淋巴细胞亚群,比较两组间T淋巴细胞亚群绝对计数与百分比的差异性;将疟疾患者组分为恶性疟组(n=33)与卵形疟组(n=3),比较两组间T淋巴细胞亚群绝对计数与百分比的差异性;将疟疾患者组分为普通型组(n=30)与重症型组(n=6),比较两组间T淋巴细胞亚群绝对计数与百分比的差异性;同时比较恶性疟患者治疗前后的T淋巴细胞亚群绝对计数与百分比的差异性。结果疟疾患者组CD45+(882.21±595.94个/μl),CD3+(636.00±390.84个/μl),CD3+CD4+(363.81±234.90个/μl)和CD3+CD8+(230.92±81.93个/μl)T淋巴细胞绝对计数值均低于健康对照组(2291.15±674.20个/μl,1680.15±510.72个/μl,934.15±317.80个/μl,615.07±280.14个/μl),差异具有统计学意义(t=7.11,7.67,6.88,4.62,均P<0.05);重症型患者组CD3+(398.36±254.74个/μl),CD3+CD4+(223.36±102.80个/μl)T淋巴细胞绝对计数值均低于普通型患者组(723.42±400.74个/μl,426.46±242.42个/μl),差异具有统计学意义(t=2.46,3.46,均P<0.05);治疗后的CD45+(2019.63±779.879个/μl),CD3+(1969.00±738.48个/μl)淋巴细胞绝对计数值较治疗前升高(1023.00±601.85个/μl,985.25±595.97个/μl),差异有统计学意义(t=3.54,3.65,均P<0.05)。其余结果差异均无统计学意义(t=0.85~2.86,均P>0.05)。结论疟原虫感染主要影响T淋巴细胞亚群绝对计数,而对T淋巴细胞亚群百分比无影响,提示T淋巴细胞亚群绝对计数在疟原虫感染检测及治疗预后具有重要临床意义。 Objective To investigate and identify clinical significance of T lymphocyte subsets change in peripheral blood of malaria patients.Methods From January to October 2020,49 patient samples from the Eighth Affiliated Hospital of Guangzhou Medical University were selected and divided into malaria patients group(n=36)and healthy control group(n=13).Their peripheral blood T lymphocyte subsets were detected by flow cytometry,and the differences in the absolute count and percentage of T lymphocyte subsets between the two groups were compared.The malaria patients were divided into falciparum malaria group(n=33)and egg malaria group(n=3).The absolute count and percentage of T lymphocyte subsets between the two groups were compared.The malaria patients were divided into common type group(n=30)and severe type group(n=6),and the differences in the absolute count and percentage of T lymphocyte subsets between the two groups were compared.At the same time,the absolute count and percentage of T lymphocyte subsets in patients with falciparum malaria before and after treatment were compared.Results The absolute values of malaria group CD45+(882.21±595.94 Per/μl),CD3+(636.00±390.84 Per/μl),CD3+CD4+(363.81±234.90 Per/μl)and CD3+CD8+(230.92±81.93 Per/μl)T lymphocyte count with-out were lower than those of healthy control group(2291.15±674.20 Per/μl,1680.15±510.72 Per/μl,934.15±317.80 Per/μl,615.07±280.14 Per/μl),and the differences were statistically significant(t=7.11,7.67,6.88,4.62,all P<0.05).The absolute value of CD3+(398.36±254.74 Per/μl)and CD3+CD4+(223.36±102.80 Per/μl)T lymphocytes in the severe malaria group were significantly lower compared with normal malaria group(723.42±400.74 Per/μl,426.46±242.42 Per/μl),the differences were statistically significant(t=2.46,3.46,all P<0.05).The absolute counts of CD45+(2019.63±779.879 Per/μl)and CD3+(1969.00±738.48 Per/μl)lymphocytes after treatment were higher than that before treatment(1023.00±601.85 Per/μl.985.25±595.97 Per/μl),and the differences were statistically significant(t=3.54,3.65,P<0.05).The other results were not statistically significant(t=0.85~2.86,all P>0.05).Conclusion Plasmodium infection mainly affects the absolute count of T lymphocyte subsets,but had no effect on the percentage of T lymphocyte subsets,suggesting that the absolute count of T lymphocyte subsets has important clinical significance in the detection of malaria infection and treatment prognosis.
