军事飞行人员血常规参考区间的建立及适用性研究

Establishment and applicability of blood routine reference intervals for military flight personnel

目的初步建立健康军事飞行人员血常规28项主要参数指标的参考区间,并进行适用性验证。方法选择2015年8月—2020年11月,在海军青岛特勤疗养中心健康体检的4854名男性军事飞行人员,采用Sysmex XN-1000五分类血细胞分析仪进行血常规检测,应用SPSS 22.0软件对数据进行统计分析,建立血常规28项主要参数的参考区间,并使用不同血常规仪器进行适用性验证。结果军事飞行人员血常规主要参数指标均为偏态分布(P<0.01),按照Dixon法去除离群值后,建立军事飞行人员血常规参考区间。将军事飞行人员血常规参数的参考区间与行业标准比较,军事飞行人员血常规主要参数中白细胞总数(4.3~10.3)×10^(9)/L、淋巴细胞百分数(24%~52%)与淋巴细胞绝对值(1.5~4.0)×10^(9)/L、单核细胞百分数(4%~11%)与单核细胞绝对值(0.3~0.8)×10^(9)/L、红细胞计数(4.5~6.0)×10^(12)/L、血红蛋白(140~178)g/L、血细胞比容(42%~52%)数值偏高,而血小板计数差异较小。本研究建立的军事飞行人员血常规参考区间,通过Sysmex XN-550及Beckman Coulter LH750全自动血液分析仪进行适用性验证,24个血常规主要参数90%以上在参考区间内。结论初步建立健康男性军事飞行人员的血常规参考区间,可为军事飞行人员的航空医学鉴定提供参考依据。

Objective To establish reference intervals of 28 main parameters of blood routine tests for healthy military flight personnel and verify their applicability.Methods From August 2015 to November 2020,4854 male military flight personnel who underwent physical examination in Qingdao Recuperation Center for Special Services were selected.Blood routine tests were performed with Sysmex XN-1000 five-classification hematology analyzers.SPSS 22.0 software was used for statistical analysis,and the reference intervals of 28 major parameters of blood routine tests were established.Different instruments for blood routine tests were used to verify the applicability.Results Skewed distribution was found in main parameters of blood routine tests for military flight personnel(P<0.01).After outliers were removed using the Dixon method,the reference intervals of blood routine tests of military flight personnel were established.Compared with the standard(WS/T 405-2012),the total number of white blood cells(4.3—10.3)×10^(9)/L,percentage of lymphocytes(24%—52%),absolute value of lymphocytes(1.5—4.0)×lO9/L,percentage of monocytes(4%—11%),absolute value of monocytes(03—0.8)×10^(9)/L,red blood cell counts(4.5~6.0)×10^(12)/L,hemoglobin(140—178)g/L and hematocrit(42%~52%)were higher,but the occupation as a military flight personnel had little influence on platelet counts.The applicability of these intervals were verified by Sysmex XN-550 and Beckman Coulter LH750 automatic hematology analyzers.Conclusion Tentative blood routine reference intervals for healthy male military flight personnel have been established,which can provide reference basis for aviation medical identification of military pilots.
