周华教授基于分期辨证论治心房颤动的经验

Professor Zhou Hua’s experience in treating atrial fibrillation based on staged dialectical theory

心房颤动是常见的心律失常之一,临床上具有较高的发病率、致残率和死亡率。周华教授认为心房颤动属本虚标实证,本虚以气血阴阳亏虚为主,标实以痰饮、瘀血为主。治疗上强调应分期论治,分初期、中期、后期三期论治。初期心之气阴两伤是其发病关键,治疗以益气养阴为主;中期心脾两虚致痰瘀互结,治疗当化痰祛瘀兼以健脾;后期伤及肾气致心肾阳虚是疾病迁延不愈和恶化的重要原因,治疗以温阳补肾。

Atrial fibrillation is one of the most common arrhythmias characterized with high morbidity,disability,and mortality in the clinical field.Professor Zhou Hua believes that atrial fibrillation is the syndrome of deficiency of root case and excess of manifestation.The deficiency of root case is mainly caused by deficiency of qi,blood,yin and yang,the excess of manifestation is mainly phlegm and blood stasis.In terms of treatment,it is emphasized that treatment should be carried out by three stages,including early stage,middle stage,and late stage.At the early stage,the heart’s qi and yin injuries are the key to its pathogenesis,and the treatment is mainly to replenish qi and nourish yin.In the middle stage,phlegm and blood stasis are caused by deficiency of heart and spleen,which should be treated by resolving phlegm,removing blood stasis,and strengthening the spleen.In the later stage,the deficiency of heart and kidney yang caused by the injury of kidney qi is an important reason for the disease to persist and worsen,treat with warming yang and tonifying kidney.
