经阴道超声引导吸胚介入治疗子宫瘢痕妊娠的临床应用

Clinical application of transvaginal ultrasound-guided embryo aspiration intervention in the treatment of uterine scar pregnancy

目的探讨子宫瘢痕妊娠采取经阴道超声引导吸胚介入治疗的临床效果。方法通过随机化原则将本院2019年1月,2021年6月接收的90例子宫瘢痕妊娠患者分为两组,对照组(n=45)给予甲氨蝶呤治疗,治疗组(n=45)行经阴道超声引导吸胚介入治疗。对比分析两组临床指标和不良反应。结果治疗后,治疗组的HCG恢复时间为(17.51±3.86)d、住院时间为(18.59±2.70)d、月经复常时间为(33.59±6.24)d比对照组的(29.75±4.14)d、(25.68±3.34)d和(42.35±5.59)d时间更短,组间数据比较差异有统计学意义(t=14.5060,P<0.05;t=11.0740,P<0.05;t=7.0143,P<0.05);且治疗组治疗后的并发症发生率仅为4.44%比对照组并发症发生率15.56%更低,差异比较具有统计学意义(χ^(2)=4.0500,P<0.05)。结论子宫瘢痕妊娠患者临床治疗中实施经阴道超声引导吸胚介入治疗效果可靠,安全性高,值得推广。

Objective To investigate the clinical effect of transvaginal ultrasound-guided embryo aspiration in the treatment of uterine scar pregnancy.Methods According to the principle of randomization,90 cases of uterine scar pregnancy received in our hospital from January 2019to June 2021 were divided into two groups.The control group(n=45)was treated with methotrexate,and the treatment group(n=45)was treated with transvaginal ultrasound-guided embryo aspiration.The clinical indicators and adverse reactions of the two groups were compared and analyzed.Results After treatment,the recovery time of HCG in the treatment group was(17.51±3.86)d,the length of hospital stay was(18.59±2.70)d,and the time of menstrual recurrence was(33.59±6.24)d than that of(29.75±4.14)d,(25.68±3.34)d and(42.35±5.59)d in the control group,and the difference between the groups was statistically significant(t=14.5060,P<0.05;t=11.0740,P<0.05;t=7.0143,P<0.05),and the complication rate after treatment in the treatment group was only 4.44%lower than that in the control group of 15.56%,and the difference was statistically significant(χ^(2)=4.0500,P<0.05).Conclusion Transvaginal ultrasound-guided embryo aspiration in the clinical treatment of uterine scar pregnancy is reliable,safe and worthy of promotion.
