腹腔镜手术中地佐辛结合罗哌卡因麻醉的应用及并发症发生率探讨

Application of dezocine combined with ropivacaine anesthesia in laparoscopic surgery and discussion on the incidence of complications

目的行腹腔镜手术治疗患者于其麻醉方案中,采用术前0.75%罗哌卡因局部浸润、术后地佐辛持续滴注麻醉,对促进患者术后康复、缓解术后疼痛程度的价值探究。方法2021.1-2023.1期间我院行腹腔镜手术治疗患者70例,行规范性分组,采用不同的麻醉方案,分别为对照组(术前0.9%氯化钠局部浸润、术后地佐辛持续滴注麻醉)、观察组(术前0.75%罗哌卡因局部浸润、术后地佐辛持续滴注麻醉)。统计组间患者麻醉效果的差异性。结果于术后各阶段评价组间疼痛程度差异,术后6h、12h、24h、48h评分值均以观察组更低,P<0.05.观察组术后下床活动、肛门排气所需时长更短,恢复速度更快,镇痛泵主动按压次数更少,P<0.05。统计两组术后并发症发生情况,观察组统计值5.71%与对照组8.57%差异较小,术后安全性相当,P>0.05.结论行腹腔镜手术治疗患者于其麻醉方案中,采用术前0.75%罗哌卡因局部浸润、术后地佐辛持续滴注麻醉,对促进患者术后康复、缓解术后疼痛程度的价值显著。

Objective To explore the value of local infiltration of 0.75%ropivacaine before operation and continuous drip anesthesia of dezocine after operation in promoting postoperative rehabilitation and relieving postoperative pain in patients undergoing laparoscopic surgery.Methods All the samples were taken from 70 patients who underwent laparoscopic surgery in our hospital from January 2021 to January 2023.The samples were divided into standardized groups,and different anesthesia schemes were adopted,namely,the control group(local infiltration of 0.9%sodium chloride before operation and continuous drip anesthesia of dezocine after operation)and the observation group(local infiltration of 0.75%ropivacaine before operation and continuous drip anesthesia of dezocine after operation).The difference of anesthesia effect between groups was counted.Results The difference of pain degree between the two groups was evaluated at different stages after operation,and the scores of 6h,12h,24h and 48h after operation were lower in the observation group(P<0.05).In the observation group,the time required for getting out of bed and anal scheduling was shorter,the recovery speed was faster,and the number of active compressions by analgesic pump was less,P<0.05.The incidence of postoperative complications in the two groups was statistically analyzed.There was little difference between the observation group(5.71%)and the control group(8.57%),and the postoperative safety was equivalent,P>0.05.Conclusion The local infiltration of 0.75%ropivacaine before operation and continuous drip anesthesia of dezocine after operation are of great value in promoting postoperative rehabilitation and relieving postoperative pain.
