基于“脑-心-肾-精室”轴探讨针刺治疗慢性前列腺炎的理论与应用

Theory and Application of Acupuncture Treatment for Chronic Prostatitis Based on‘Brain-Heart-Kidney-Essence Chamber’Axis

慢性前列腺炎为常见的男性附属性腺疾病,具有临床表现复杂、发病率高的特点。本文旨在基于“脑-心-肾-精室”轴探讨针刺治疗慢性前列腺炎的思路。“脑-心-肾-精室”轴认为脑、心、肾与精室在生理上环环相扣,以督脉为纽带,以精血为基础,以神明为统领,以少阴为枢机,促进精室功能协调有度;在病理方面常以脑心失调、脑肾失济、心肾失交进而致精室失用为主。本文通过分析脑、心、肾、精室彼此间的经脉络属关系,在此基础上提倡慢性前列腺炎的治疗需要以肾为核心,以“脑、心共主神明”为依据,将整体与局部同治,心身同调为治疗原则,拟定止痛、改善排尿、调节性功能、调神等针法并结合“脑-心-肾-精室”轴的辨治思路确立相应治法,丰富选穴方案,为慢性前列腺炎的中医治疗提供参考。

Chronic prostatitis(CP)is a common accessory gonadal disease in males.It has the characteristics of complex clinical manifestations and high incidence rate.This study aims to explore the idea of acupuncture in treatment of CP based on the axis of‘Brain-Heart-Kidney-Essence chamber’.It is considered that Brain,Heart,Kidney and Essence chamber are physiologically linked,with Du meridian as the link,essence and blood as the basis,Shen as the commander and Shaoyin as the pivot,in promoting the functional coordination of Essence chamber.In pathology,the dysfunction of Essence chamber is caused by Brain-Heart disorder,Brain-Kidney disharmony and Heart-Kidney non-communication.The relationship of meridians and collaterals among Brain,Heart,Kidney and Essence chamber was analyzed in this study.On this basis,it is advocated that the treatment of CP needs to take Kidney as the focus,with the characteristics of‘Brain and Heart jointly control Shen’,treating the overall and the local simultaneously,regulating physic body and Shen together as the treatment principle,formulating acupuncture treatment methods of pain relief,improving urination,regulating sexual function and regulating Shen,in combination with the syndrome differentiation and treatment idea of‘Brain-Heart-Kidney-Essence chamber’axis,so as to enrich the acupoints selection scheme and provide reference for TCM therapies in treatment of CP.
