医院社区一体化管理模式对严重精神分裂症患者肇事肇祸行为、精神状态及社会功能的影响

Effects of hospital-community integrated management mode on violent behaviors,mental state and social function in patients with severe schizophrenia

目的探讨医院社区一体化管理模式对严重精神分裂症患者肇事肇祸行为、精神状态及社会功能的影响。方法选取2020年1月至2022年2月在福建省泉州市第三医院确诊的92例严重精神分裂症患者作为研究对象,根据患者入院时间进行分组,选取2020年1月至2021年1月确诊的45例患者为对照组,选取2021年2月至2022年2月确诊的47例患者为观察组。对于对照组按照常规方式进行管理,对于观察组实施医院社区一体化管理的方式进行管理,干预6个月。干预前后,分别观察两组肇事肇祸行为、精神状态及社会功能。结果两组干预前的社会功能缺陷筛选量表(SDSS)评分和阳性和阴性症状量表(PANSS)评分比较,差异无统计学意义(P>0.05);观察组和对照组干预6个月后的SDSS评分、PANSS评分均低于本组干预前,差异有统计学意义(P<0.05),观察组患者干预6个月后的SDSS评分和PANSS评分低于对照组,差异有统计学意义(P<0.05);两组干预前的肇事肇祸率比较,差异无统计学意义(P>0.05);观察组干预6个月后的肇事肇祸率低于本组干预前,差异有统计学意义(P<0.05),对照组干预6个月后的肇事肇祸率与本组干预前比较,差异无统计学意义(P>0.05);观察组患者干预6个月后的肇事肇祸率低于对照组,差异有统计学意义(P<0.05)。结论在严重精神分裂症患者接受治疗时给予医院社区一体化的管理模式,对于降低其肇事肇祸行为发生率、改善精神状态及社会功能有重要作用。

Objective To explore the effects of hospital-community integrated management mode on violent behaviors,mental state and social function in patients with severe schizophrenia.Methods A total of 92 patients with severe schizophrenia confirmed in Quanzhou Third Hospital were selected as the study objects,the patients were grouped according to the admission time,45 patients diagnosed from January 2020 to January 2021 were selected as the control group,and 47 patients diagnosed from February 2021 to February 2022 were selected as the observation group.The control group was given routine management,while observation group was given hospital-community integrated management for 6 months.Before and after intervention,violent behaviors,mental state and social function in both groups were observed.Results There was no significant difference in social disability screening schedule(SDSS)score or positive and negative syndrome scale(PANSS)score between the two groups before intervention(P>0.05).After 6 months of intervention,SDSS and PANSS scores in both groups were lower than those before intervention,and SDSS and PANSS scores in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of violent behaviors between the two groups before intervention(P>0.05).After 6 months of intervention,incidence of violent behaviors in observation group was lower than that before intervention,and the difference was statistically significant(P<0.05).In control group,there was no significant difference in the incidence of violent behaviors before and after 6 months of intervention(P>0.05).After 6 months of intervention,incidence of violent behaviors in observation group was lower than that in control group,and the difference was statistically significant(P<0.05).Conclusion The hospital-community integrated management mode plays important roles in reducing the incidence of violent behaviors and improving mental state and social function in patients with severe schizophrenia.
