触发阈值为30 HU联合多期双流注射45 mL对比剂对大体重人群行头颈部联合CTA扫描的分析

Analysis of Head and Neck Combined CTA Scanning in Heavy Weight Population with Trigger Threshold of 30 HU and Multi-phase Double-flow Injection of 45 mL Contrast Agent

目的 分析触发阈值为30 HU联合多期双流注射45 mL对比剂在大体重人群行头颈部联合CT血管造影(CT angiography,CTA)扫描中的应用价值。方法 选取2020年1月—2022年3月期间于仪征市人民医院行头颈联合CTA扫描患者66例为研究对象,依照随机投掷法分为对照组(n=33)与观察组(n=33)。对照组:注射对比剂55 mL+触发阈值设为100 HU;观察组:触发阈值30 HU+多期双流注射45 mL,对比两组动脉强化程度、头颈部各段动脉成像效果。结果 两组主动脉弓、双侧颈总动脉、颈内动脉、双侧大脑中动脉等强化程度对比,观察组更低,差异有统计学意义(t=12.397、12.819、9.705、11.968,P<0.05);两组头颈部各段动脉成像优秀率、良好率、差率比较,差异无统计学意义(Z=0.548,P>0.05);对照组成像效果评分为(1.31±0.45)分、观察组为(1.30±0.47)分,两组比较差异无统计学意义(t=0.088,P>0.05)。结论 低触发阈值+多期双流注射45 mL对比剂,在大体重人群头颈部联合CTA扫描患者中同样可行,有推广价值。

Objective To analyze the application value of a trigger threshold of 30 HU combined with multi-phase double-flow injection of 45 mL contrast agent in heavy weight patients undergoing head and neck combined CT aiography(CTA)scanning.Methods 66 patients who underwent head and neck combined with CTA scanning in Yizheng People´s Hospital from January 2020 to March 2022 were selected as the research object and divided into control group(n=33)and observation group(n=33)according to random throwing method.Control group:55 mL of contrast agent was injected+triggering threshold was set to 100 HU.Observation group:trigger threshold 30 HU+multi-phase double-flow injection 45 mL.The degree of arterial enhancement and the results of the imaging effect of each segment of head and neck arteries between the two groups was compared.Results The enhancement degree of aortic arch,bilateral common carotid artery,internal carotid artery and bilateral middle cerebral artery was lower in the observation group,the difference was statistically significant(t=12.397,12.819,9.705,11.968,P<0.05);there was no statistically significant difference in the excellent rate,good rate and poor rate of head and neck artery imaging between the two groups(Z=0.548,P>0.05);the imaging effect scores of the control group and the observation group were(1.31±0.45)points and(1.30±0.47)points respectively,there was no statistically significant difference(t=0.088,P>0.05).Conclusion Low trigger threshold plus multi-phase double-flow injection of 45mL contrast agent is also feasible in heavy weight patients with head and neck combined with CTA scanning,and has the value of promotion.
