连续性血液净化治疗(CRRT)在脓毒症诱发多脏器功能障碍综合征患者中的疗效

Efficacy of Continuous Renal Replacement Therapy(CRRT)in Patients with Sepsis-induced Multiple Organ Dysfunction Syndrome

目的 探讨对脓毒症诱发多脏器功能障碍综合征患者实施连续性血液净化(continuous renal replacement therapy, CRRT)治疗的临床效果。方法 选取2018年4月—2021年7月常州市武进人民医院收治的40例脓毒症诱发多脏器功能障碍综合征患者,随机分为常规组(给予常规治疗)和研究组(常规组基础上给予CRRT治疗),各20例,比较两组患者乳酸清除率、序贯器官衰竭估计评分(SOFA评分)、呼吸机使用时间、生存率。结果 研究组乳酸清除率(65.29±8.53)%明显高于常规组,呼吸机使用时间(7.35±3.25)d明显短于常规组,差异有统计学意义(t=7.113、3.437,P<0.05);治疗前,两组SOFA评分对比,差异无统计学意义(P>0.05),治疗后,研究组SOFA评分(7.65±1.56)分明显低于常规组,差异有统计学意义(t=11.298,P<0.05);研究组生存率为95.00%,高于常规组的65.00%,差异有统计学意义(P<0.05)。结论 CRRT方式的有效应用,可有效改善患者乳酸清除率、缩短呼吸机使用时间,有效提高生存率,可促进患者良好预后。

Objective To investigate the clinical effects of continuous renal replacement therapy(CRRT)in patients with sepsis-induced multiple organ dysfunction syndrome.Methods Forty patients with sepsis-induced multiple organ dysfunction syndrome admitted to Changzhou Wujin People´s Hospital from April 2018 to July 2021 were selected.The patients were randomly divided into a conventional group(given conventional treatment)and a study group(CRRT was given on the basis of the conventional group),with 20 cases in each group.The lactate clearance rate,Sequential Organ Failure Assessment Score(SOFA score),duration of ventilator use,survival rate were compared between the two groups.Results The lactate clearance rate(65.29±8.53)%in the study group was significantly higher than that in the conventional group,and the use time of the ventilator(7.35±3.25)d was significantly shorter than that in the conventional group,the difference was statistically significant(t=7.113,3.437,P<0.05).Before treatment,there was no statistically significant difference in the SOFA score between the two groups(P>0.05).After treatment,the SOFA score of the study group(7.65±1.56)points was significantly lower than that of the conventional group,the difference was statistically significant(t=11.298,P<0.05).The survival rate of the study group was 95.00%,higher than that of the conventional group(65.00%),the difference was statistically significant(P<0.05).Conclusion The effective application of CRRT treatment modality can effectively improve the lactate clearance rate,shorten the time of ventilator use and effectively improve the survival rate of patients,and,can promote good prognosis of patients.
