对腹部创伤患者应用CT诊断的价值及意义研究

Study on the Value and Significance of Applying CT Diagnosis to Patients with Abdominal Trauma

目的 探究腹部创伤患者诊断中螺旋CT检查的应用价值及意义。方法 选取2019年2月—2021年2月南方医科大学顺德医院附属杏坛医院收治的52例疑似腹部创伤患者开展研究。于术前分别进行数字X线摄影检查、螺旋CT检查,对比两种检查方式的图像质量;同时以临床诊断结果为金标准,比较两种检查方式的诊断效能(诊断准确性、敏感度、特异性)。结果 螺旋CT检查图像质量高于数字X线摄影检查,差异有统计学意义(P<0.05);以临床诊断结果为金标准,螺旋CT检查对疾病的诊断准确性(92.31%)、敏感度(86.36%)均较数字X线摄影检查(71.15%、59.09%)更高,差异有统计学意义(P=0.007、0.031);螺旋CT检查的特异度(96.67%)与数字X线摄影检查(80.0%)比较,差异无统计学意义(P=0.063)。结论 螺旋CT检查可保证图像质量,提高对腹部创伤患者的诊断效果,值得推广。

Objective To explore the application value and significance of spiral CT in the diagnosis of abdominal trauma patients.Methods From February 2019 to February 2021,52 patients with suspected abdominal trauma admitted to Xingtan Hospital,Shunde Hospital of Southern Medical University were selected for the study.Digital radiography and spiral CT were performed before operation to compare the image quality of the two methods;at the same time,the diagnostic efficacy(diagnostic accuracy,sensitivity and specificity)of the two methods were compared with the clinical diagnosis results as the gold standard.Results The image quality of spiral CT was higher than that of digital radiography,and the difference was statistically significant(P<0.05).Taking clinical diagnosis results as the gold standard,the diagnostic accuracy(92.31%)and sensitivity(86.36%)of spiral CT were higher than those of digital radiography(71.15%)and(59.09%),and the difference was statistically significant(P=0.007,0.031).Compared with the specificity of spiral CT(96.67%)and digital radiography(80.0%),the difference was not statistically significant(P=0.063).Conclusion Spiral CT can ensure the image quality and improve the diagnostic effect of abdominal trauma patients,which is worth popularizing.
