经剑突下胸腔镜手术在胸腺瘤患者治疗中的临床效果分析

Clinical Effectiveness Analysis of Subxiphoid Thoracoscopic Surgery in the Treatment of Patients with Thymoma

目的 探讨经剑突下胸腔镜手术在胸腺瘤治疗中的应用效果。方法 选取2019年2月—2021年6月茂名市中医院收治的38例胸腺瘤患者,按随机双盲法分为剑突组(n=19)与对照组(n=19)。对照组给予经肋间胸腔镜手术治疗,剑突组给予经剑突下胸腔镜手术治疗。比较两组患者的手术情况、术后恢复情况与生活质量水平及并发症发生情况。结果 剑突组引流时间(3.57±1.02)d、出血量(118.63±31.56)mL、手术耗时(106.42±19.79)min少于对照组,差异有统计学意义(t=3.961、4.919、2.256,P<0.05);剑突组使用抗生素天数、切口长度、住院天数比对照组短,VAS评分比对照组低,SF-36评分比对照组高,差异有统计学意义(P<0.05)。结论 经剑突下胸腔镜手术治疗的手术创伤小,胸腺瘤患者术后恢复速度快,术后并发症发生风险小,治疗安全性较高,该手术具有较高的临床应用价值。

Objective To explore and analyze the effect of transcatheter subxiphoid thoracoscopic surgery in the treatment of thymoma.Methods Thirty-eight patients with thymoma admitted to Maoming Hospital of Traditional Chinese Medicine from February 2019 to June 2021 were selected and were divided into the xiphoid process group(n=19)and the control group(n=19)by randomized double-blind method.The control group was treated with intercostal thoracoscopic surgery,and the xiphoid process group was treated with transcatheter subxiphoid thoracoscopic surgery.The operation,postoperative recovery,quality of life and complications of the two groups were compared.Results The time to drainage(3.57±1.02)d,bleeding volume(118.63±31.56)mL,and operative elapsed time(106.42±19.79)min in the xiphoid process group were less than those in the control group,the difference was statistically significant(t=3.961,4.919,2.256,P<0.05).The number of days with antibiotics,incision length,and hospital days of the xiphoid process group were shorter than that of the control group,VAS scores of the xiphoid process group was lower than that of the control group,and SF-36 scores was higher than that of the control group,the difference was statistically significant(P<0.05).Conclusion Transcatheter subxiphoid thoracoscopic surgery is less traumatic,and patients with thymoma have faster postoperative recovery,less risk of postoperative complications,and higher treatment safety,and the procedure has high clinical application value.
