三维可视化技术联合微波消融术治疗肝脏血管瘤患者的效果

摘要 目的:观察三维可视化技术联合微波消融术治疗肝脏血管瘤患者的效果。方法:选取2020年4月至2022年4月该院收治的80例肝脏血管瘤患者进行前瞻性研究,按照随机数字表法将其分为研究组和对照组各40例。对照组采用微波消融术治疗,研究组在对照组基础上联合三维可视化技术治疗,比较两组围术期指标水平、消融指标水平、手术前后肝功能指标[丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、白蛋白(ALB)、总胆红素(TBiL)]水平、疼痛程度[视觉模拟评分法(VAS)]评分和临床疗效。结果:两组手术时间、住院时间比较,差异均无统计学意义(P>0.05);研究组消融能量低于对照组,消融时间短于对照组,差异均有统计学意义(P<0.05);两组消融点数比较,差异无统计学意义(P>0.05);术后3 d,两组ALT、AST、TBiL水平均高于术前,但研究组低于对照组,两组ALB水平均低于术前,但研究组高于对照组,差异有统计学意义(P<0.05);术后12、24、48 h,两组VAS评分均低于术前,且研究组低于对照组,差异有统计学意义(P<0.05);研究组治疗总有效率为80.00%(32/40),高于对照组的40.00%(16/40),差异有统计学意义(P<0.05)。结论:三维可视化技术联合微波消融术治疗肝脏血管瘤患者可提高治疗总有效率,改善消融指标和肝功能指标水平,降低VAS评分,效果优于单纯微波消融术治疗。 Objective:To observe effects of 3D visualization combined with microwave ablation in treatment of hepatic hemangioma.Methods:A prospective study was conducted on 80 patients with hepatic hemangioma admitted to the hospital from April 2020 to April 2022.They were divided into study group and control group according to the random number table method,40 patients in each group.The control group was treated with microwave ablation,while the study group was treated with 3D visualization technology on the basis of that of the control group.The perioperative index levels,the ablation index levels,the liver function index levels[alanine aminotransferase(ALT),aspartate aminotransferase(AST),albumin(ALB),total bilirubin(TBiL)],the pain degree[visual analogue scale(VAS)]score,and the clinical efficacy were compared between the two groups.Results:There were no significant differences between the two groups in the operation time and the hospitalization time(P>0.05).The ablation energy of the study group was lower than that of the control group,the ablation time was shorter than that of the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the number of ablation points between the two groups(P>0.05).3 days after the surgery,the levels of ALT,AST and TBiL in the two groups were higher than those before the surgery,but those in the study group were lower than those in the control group;the level of ALB in both groups was lower than that before the surgery,but that in the study group was higher than that in the control group;and the differences were statistically significant(P<0.05).12,24 and 48 hours after the surgery,the VAS scores of the two groups were lower than those of the control group,and the differences were statistically significant(P<0.05).The total effective rate of treatment in the study group was 80.00%(32/40),which was higher than 40.00%(16/40)in the control group,and the difference was statistically significant(P<0.05).Conclusions:3D visualization combined with microwave ablation can improve the total effective rate of treatment,improve the levels of ablation indexes and liver function indexes,and reduce the VAS score.Moreover,it is superior to single microwave ablation.
