身痛逐瘀汤在痰瘀互阻型类风湿关节炎中的应用研究

Application of Shentong Zhuyu Decoction in Rheumatoid Arthritis of Phlegm and Blood Stasis Mutual Resistance Type

目的:探究身痛逐瘀汤治疗类风湿关节炎(痰瘀互阻型)中的临床效果。方法:选取本院2021年1月—2022年1月诊治的类风湿关节炎(痰瘀互阻型)患者57例为研究对象,利用随机数字表法分为研究组29例和对照组28例,分别使用身痛逐瘀汤、来氟米特进行口服治疗。比较两组治疗前后体征指标变化(关节压痛数、关节肿胀数、晨僵时间)、实验室指标变化[血小板计数(PLT)、红细胞沉降率(ESR)、C-反应蛋白(CRP)、类风湿因子(RF)]以及治疗后的不良反应发生率。结果:两组治疗后关节压痛数、关节肿胀数、晨僵时间低于治疗前,且研究组优于对照组,差异有统计学意义(P<0.05);两组治疗后PLT、IL-6、CRP、RF水平低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05);研究组不良反应发生率为3.45%,低于对照组的28.57%,差异有统计学意义(P<0.05)。结论:采用身痛逐瘀汤治疗痰瘀互阻型类风湿关节炎患者具有显著疗效,可改善患者体征指标,降低类风湿性关节炎相关指标,且安全性高,值得临床推广应用。

Objective:To explore the clinical effect of Shentong Zhuyu Decoction in the treatment of rheumatoid arthritis with phlegm and blood stasis type.Methods:A total of 57 patients with rheumatoid arthritis(phlegm and blood stasis mutual resistance type)diagnosed and treated in our hospital from January 2021 to January 2022 were selected as the research objects.They were divided into the study group of 29 cases and the control group of 28 cases by random number table method.Oral treatment with Shengtong Zhuyu decoction and leflunomide were used respectively.The changes of physical signs(joint tenderness,morning stiffness time,and joint swelling)and laboratory indexes[platelet count(PLT),interleukin-6(IL-6),C-reactive protein(CRP),rheumatoid arthritis(RF)]and the incidence of adverse reactions after treatment were compared between the two groups before and after treatment.Results:The number of joint tenderness,joint swelling and morning stiffness in the two groups after treatment were lower than those before treatment,and those in the study group were better than the control group,the difference was statistically significant(P<0.05).After treatment,PLT,IL-6,CRP and RF levels in the two groups were lower than those before treatment,and those in the study group were lower than those in the control group,with significant difference(P<0.05).The incidence of adverse reactions in the study group was 3.45%,which was lower than 28.57%in the control group,and the difference was statistically significant(P<0.05).Conclusion:The use of Shentong Zhuyu decoction in the treatment of rheumatoid arthritis patients with phlegm and blood stasis mutual resistance has significant curative effect,and can improve the patient’s physical indicators,reduce rheumatoid arthritis-related indicators,and have high safety,which is worthy of clinical application.
