冠心病患者焦虑检出情况及其对心电图的影响

Anxiety detection in patients with coronary heart disease and its influence on electrocardiogram

目的探讨冠心病患者焦虑检出情况及对心电图的影响。方法选择2018年1月~2022年1月收治的300例冠心病患者为研究对象,采用焦虑自评量表(SAS)评估患者焦虑情绪,并设为焦虑组与非焦虑组。监测两组患者24h动态心电图,比较室性早搏、房性早搏等异常心电图发生情况,比较两组患者心率变异性指标。结果300例冠心病患者焦虑检出136例,焦虑组房室传导阻滞、室性早搏、房性早搏、室性心动过速、短阵房性心动过速发生率显著高于非焦虑组(P<0.05);焦虑组差值>50ms相邻正常周期个数比(PNN50)、HRV NN间期标准差(SDNN)、相邻NN间期差值均方根(rMSSD)等心率变异性指标显著低于非焦虑组(P<0.05)。结论冠心病患者焦虑检出率较高,焦虑情绪会增加房性早搏、室性早搏等异常心电图发生率,且影响患者冠心病患者心率变异性。

Objective To investigate the detection of anxiety in patients with coronary heart disease and its influence on electrocardiogram.Methods A total of 300 patients with coronary heart disease from January 2018 to January 2022 were selected as the research objects.The anxiety self-rating scale(SAS)was used to evaluate the anxiety of the patients,and they were divided into anxiety group and non-anxiety group.The 24-h dynamic electrocardiogram was monitored in the two groups.the occurrence of abnormal ECG such as premature ventricular contraction and premature atrial contraction were compared.The heart rate variability indexes of the two groups were compared.Results 136 cases of anxiety were detected in 300 patients with coronary heart disease.The incidence of atrioventricular block,premature ventricular contraction,premature atrial contraction,ventricular tachycardia and short paroxysmal atrial tachycardia in the anxiety group was significantly higher than that in the non-anxious group(P<0.05).The heart rate variability indexes such as the proportion of the number of two adjacent normal cycles(PNN50),HRV Standard deviation of NN intervals(SDNN)and the root mean square difference ofthe adjacent NN interval(rMSSD)in the anxiety group were significantly lower than those in the non-anxiety group(P<0.05).Conclusion The detection rate of anxiety in patients with coronary heart disease is high,and anxiety will increase the incidence of abnormal ECG such as premature ventricular contraction and premature atrial contraction,and affect the heart rate variability of patients with coronary heart disease.
