高胆红素血症对新生儿肝功能的影响探讨

摘要 目的 探讨高胆红素血症对新生儿肝功能的影响。方法 选取商丘市妇幼保健院2020年2月—2022年1月收治的高胆红素血症患儿80例,并取同期健康新生儿80例作为对照组,均开展肝功能指标检查,同时对高胆红素血症治疗前后的肝功能指标变化进行记录,分析高胆红素血症对新生儿肝功能产生的影响。结果 高胆红素血症组的谷草转氨酶(67.53±11.65)U/L、碱性磷酸酶(280.65±65.47)U/L、谷氨酰转肽酶水平(121.45±36.65)U/L,均高于对照组,差异有统计学意义(P<0.05);两组谷丙转氨酶、白蛋白水平差异无统计学意义(P>0.05);高胆红素血症组治疗后谷草转氨酶(39.10±5.24)U/L、碱性磷酸酶[(155.68±28.54)U/L]与谷氨酰转肽酶水平[(94.10±22.52)U/L]均低于治疗前,差异有统计学意义(P<0.05),谷丙转氨酶治疗后亦低于治疗前,但对比无统计学意义(P>0.05)。结论 高胆红素血症通常不会对肝细胞产生损害,但会对肝功能产生影响,损伤具备可逆性,通过尽早开展积极治疗,可使患儿的肝功能损伤得到改善。临床上在治疗高胆红素血症过程中,需做好患儿肝功能监测。 Objective To investigate the effects of hyperjaundicinemia on neonatal liver function. Methods Totally 80 cases of children with hyperbilirubinemia admiteed to Shangqiu Maternity and Child Care Hospital from February 2020 to January 2022 were sehected and 80 healthy neonates were selected as the control group. Liver function indexes were examined in all cases. Meanwhile, the changes of liver function indexes before and after hyperbilirubinemia were recorded to analyze the effects of hyperjaundicinemia on neonatal liver function.Results The levels of aspartate aminotransferase(67.53±11.65)U/L, alkaline phosphatase(280.65±65.47)U/L and glutamyl transpeptidase(121.45±36.65)U/L in hyperictericinemia group were higher than those in control group, with statistical significance(P<0.05). There were no significant differences in alanine transaminase and albumin levels between the two groups(P>0.05). After treatment, the levels of as partate aminotransferase(39.10±5.24) U/L, alkaline phosphatase [(155.68±28.54) U/L] and glutamyl transpeptidase [(94.10±22.52) U/L] in hyperictericinemia group were lower than those before treatment, and the difference was statistically significant(P<0.05). A lanine transamirase was also lower after treatment than before treatment, but there was no statistical significance(P>0.05). Conclusion Hyperjaundicinemia usually does not cause damage to liver cells, but it can affect liver function. The damage is reversible, through early and active treatment, the liver function damage can be improved in children. In clinical treatment of hyperictericinemia, it is necessary to monitor the liver function of children.
