多模态MRI评估子宫内膜癌肌层浸润深度及宫颈侵犯程度和术前分期的价值

Value analysis of multimodal MRI in evaluation of depth of muscular invasion, degree of cervical invasion and preoperative staging of endometrial carcinoma

目的 将多模态MRI应用子宫内膜癌(endometrial cancer, EC)肌层浸润深度、宫颈侵犯程度及术前分期的评估工作中,对其评估价值进行探究与分析。方法 于2021年3月—2022年3月选取永城市人民医院收治的78例子宫内膜癌患者,分别以CT扫描(对照组)与多模态MRI成像(观察组)的方式分析EC肌层浸润深度、宫颈侵犯程度及术前分期的评估效果。结果 观察组肌层浸润深度评估在敏感度、特异度、准确度方面分别为94.23%、92.31%、93.59%,均高于对照组(P<0.05);观察组宫颈侵犯程度的评估敏感度、特异度、准确度方面分别为98.48%、90%、94.87%,均高于对照组(P<0.05);对照组在CT扫描下的术前分期诊断情况为真ⅠA期(28例)、真ⅠB期(18例)、真Ⅱ期(9例)、真Ⅲ期(6例)、Ⅳ期(3例);观察组在多模态MRI的术前分期诊断情况为真ⅠA期(30例)、真ⅠB期(20例)、真Ⅱ期(11例)、真Ⅲ期(7例)、Ⅳ期(4例),观察组诊断准确率为92.31%,高于对照组的82.05%(P<0.05)。结论 多模态MRI的诊断方式在评估EC患者肌层浸润深度、宫颈侵犯程度及术前分期方面可以发挥出极为显著的价值。

Objective To explore and analyze the evaluation value of multimodal MRI in the evaluation of the depth of muscular invasion, the degree of cervical invasion and preoperative staging of endometrial cancer.Methods From March 2021 to March 2022, 78 patients with endometrial carcinoma were selected for the study, and the evaluation effect of EC muscular invasion depth, cervical invasion degree and preoperative staging were analyzed by CT scanning and multimodal MRI imaging respectively.Results The sensitivity, specificity and accuracy of muscle layer infiltration depth assessment in the observation group were 94.23%, 92.31% and 93.59%, respectively, which were higher than those of the control group(P<0.05). The sensitivity, specificity and accuracy of cervical invasion evaluation in the observation group were 98.48%, 90% and 94.87%, respectively, which were higher than those in the control group(P<0.05). In the control group, preoperative staging under CT scan was true Ⅰ stage A(28 cases), true Ⅰ stage B(18 cases), true Ⅱ stage(9 cases), true Ⅲ stage(6 cases), and stage Ⅳ stage(3 cases). The preoperative staging of multimodal MRI in the observation group was true Ⅰ stage A(30 cases), true Ⅰ stage B(20 cases), true Ⅱ stage(11 cases), true Ⅲ stage(7 cases), and stage Ⅳ stage(4 cases). The diagnostic accuracy of the observation group was 92.31% higher than that of the control group(82.05%, P<0.05).Conclusion The diagnostic method of multimodal MRI can play a significant role in evaluating the depth of muscular invasion, cervical invasion and preoperative staging of EC patients.
