DECT处理技术在痛风性关节炎尿酸盐结晶沉积诊断中的应用

Application of DECT in the diagnosis of urate crystal deposition in gouty arthritis

目的 探究DECT处理技术在痛风性关节炎尿酸盐结晶沉积诊断中的应用。方法 选择2019年6月到2021年3月因关节肿痛在院就诊的180例患者,符合纳入和排除标准后剩余146例进行研究,采用回顾性分析的方法,所有患者进行穿刺活检确诊痛风性关节炎的情况,然后采用DECT扫描进行有无尿酸盐结晶沉积的检测,比较两组患者的一般资料、尿酸盐结晶沉积的效能。结果 在本研究中,通过穿刺活检结果确定116例痛风关节炎患者,通过DECT扫描检测是否有尿酸盐结晶沉积,通过对有沉积和无沉积的患者进行年龄、体重等一般资料的比较,P>0.05,没有统计学差异。对患者的血尿酸情况进行比较,无尿酸盐结晶沉积患者的血尿酸(5.92±0.59)mg/dl明显低于尿酸盐结晶沉积患者的血尿酸(9.09±0.87)mg/dl,P<0.05,差异具有统计学意义。继而通过敏感度、准确度等的数据比较,通过表格和计算的百分比发现,DECT、血尿酸比DECT联合血尿酸明显低,并且P<0.05,差异具有统计学意义。结论 血尿酸和DECT对痛风性关节炎患者尿酸盐结晶沉积能提供好的诊断,对治疗有重大的意义。

Objective To explore the application of DECT in the diagnosis of urate crystal deposition in gouty arthritis.Methods A total of 180 patients with joint swelling and pain in our hospital from January to December 2020 were selected.After meeting the inclusion and exclusion criteria, the remaining 146 patients were studied.By retrospective analysis, all patients underwent puncture biopsy to diagnose gouty arthritis, and then DECT scanning was used to detect the presence or absence of urate crystal deposition.The general data and effectiveness of urate crystal deposition of the two groups were compared.Results In this study, 116 patients with gout arthritis were determined by biopsy results.DECT scanning was used to detect whether there was urate crystal deposition.Through the comparison of age, weight and other general data between patients with and without deposition, there was no statistical difference(P>0.05).Comparing the blood uric acid of patients, the blood uric acid of patients without urate crystal deposition was(5.92±0.59)mg/dl, which was significantly lower than that of patients with urate crystal deposition(9.09±0.87)mg/dl(P<0.05).Then, by comparing the data of sensitivity and accuracy, through the table and calculated percentage, it was found that DECT and blood uric acid were significantly lower than DECT combined with blood uric acid(P<0.05),the difference was statistically significant.Conclusion Serum uric acid and DECT can provide a good diagnosis of urate crystal deposition in patients with gouty arthritis and are of great significance for treatment.
