补阳还五汤加减联合针灸治疗后循环缺血性眩晕的临床研究

Clinical Study of modified Buyang Huanwu Decoction Combined with Acupuncture in the Treatment of Posterior Circulation Ischemic Vertigo

目的 探讨补阳还五汤加减联合针灸治疗后循环缺血性眩晕的效果。方法 选择2020年3月至2021年10月我院收治的88例后循环缺血性眩晕患者为研究对象,按照随机数字表法分成对照组与观察组,每组各44例。对照组采用甲磺酸倍他司汀片与盐酸氟桂利嗪胶囊治疗,观察组在对照组基础上采用补阳还五汤联合针灸治疗,比较两组患者的临床疗效、治疗前后血液流变学指标、氧化应激指标及不良反应情况。结果 观察组的治疗总有效率较对照组高,两组比较差异有统计学意义(P<0.05)。治疗后,观察组的全血黏度、血浆黏度及红细胞压积均低于对照组,椎动脉平均血流速度及基底动脉平均血流速度均高于对照组,差异均有统计学意义(均P<0.05)。治疗后,观察组的一氧化氮、超氧化物歧化酶水平均高于对照组,丙二醛低于对照组,组间比较差异均有统计学意义(均P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论 补阳还五汤加减联合针灸治疗后循环缺血性眩晕能有效改善患者血液流变学状况和氧化应激程度,提高治疗效果,且具有较高的安全性,值得临床推广。

Objective To study the effect of modified Buyang Huanwu Decoction combined with acupuncture in the treatment of patients with posterior circulation ischemic vertigo.Methods 88 patients with posterior circulation ischemic vertigo in our hospital were selected March 2020 to October 2021,who were randomly divided into control group and observation group according to the random number table method,with 44 cases in each group.The patients of control group were treated with Betahistine Mesylate Tablets and Flunarizine Hydrochloride Capsules,the patients of observation group were treated with Buyang Huanwu Decoction combines with acupuncture based on the treatment of control group.The clinical efficacy,hemorheological indexes,oxidative stress indexes and adverse reactions of two groups were compared before and after treatment.Results The total effective rate of observation group was higher than that of control group,there was significant difference between two groups(P<0.05).After treatment,the whole blood viscosity,plasma viscosity and hematokrit of observation group were lower than those of control group,and the average blood flow velocity of vertebral artery and basilar artery were higher than those of control group,there was significant difference(P<0.05).After treatment,the levels of nitric oxide and superoxide dismutase in the observation group were higher than those in the control group,and malondialdehyde was lower than that in the control group,the difference between two groups was statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between two groups(P>0.05).Conclusion The modified Buyang Huanwu Decoction Combined with acupuncture in the treatment of posterior circulation ischemic vertigo can improve the blood flow and the degree of oxidative stress in patients with posterior circulation ischemic vertigo,and has high application safety,which is worthy of recommendation.
