富血小板血浆联合米诺地尔酊治疗雄激素性脱发疗效观察

Clinical Effect of Platelet-rich Plasma Combined with Minoxidil Tincture on Androgenetic Alopecia

目的:观察富血小板血浆(Platelet-richplasma,PRP)联合米诺地尔酊治疗雄激素性脱发(Androgenetic alopecia,AGA)的临床疗效及安全性。方法:将入选的60例患者按就诊顺序随机分为治疗组和观察组,每各30例。治疗组给予PRP头皮注射,每月1次,共计6次,同时联合米诺地尔酊外涂,2次/天,共治疗6个月;对照组仅给予米诺地尔酊外涂。比较治疗前、治疗3个月及6个月后医生视觉主观评分及皮肤镜下毛发密度,以评判临床疗效。结果:治疗后,医生视觉主观评估两组毛发密度及皮肤镜检测终毛密度和毛发密度均较治疗前有显著增加(P<0.05),并且随着疗程的延长疗效越好。治疗组治疗3个月、6个月后医生评分疗效均优于对照组(P<0.05)。治疗3个月、6个月后,治疗组终毛密度及毛发密度均高于对照组,差异均有统计学意义(P<0.05),但毳毛密度两组比较差异无统计学意义。结论:PRP联合米诺地尔酊治疗AGA起效快、疗效强,对于单用米诺地尔疗效不显著或不耐受,以及无法口服用药者是一个有效的辅助治疗方法。

Objective To observe the clinical effi cacy and safety of platelet-rich plasma combined with minoxidil tincture in the treatment of androgenetic alopecia.Methods The 60 selected patients were randomly divided into treatment group and observation group according to the order of treatment,30 cases each.The treatment group was given a platelet-rich plasma scalp injection once a month for a total of 6 times,and combined with minoxidil tincture external application,twice a day,the treatment lasted for 6 months.The control group was only given minoxidil tincture.The doctors'subjective visual scores and dermoscopic assessment of hair density were compared before treatment,3 months after treatment and 6 months after treatment to evaluate the clinical effi cacy.Results After treatment,there was a signifi cant increase(P<0.05)in the doctors'subjective visual scores and dermoscopic assessment of terminal hair density and hair density in both groups compared to pre-treatment,and the effi cacy increased as the treatment course was extended.The treatment group had better effi cacy than the control group in terms of doctors'score after 3 and 6 months of treatment(P<0.05).After 3 and 6 months of treatment,both terminal hair density and hair density in the treatment group were higher than in the control group,and the diff erences were statistically significant(P<0.05).There was no statistically signifi cant diff erence in the density of vellus hair between the two groups.Conclusion PRP combined with minoxidil tincture in the treatment of AGA has a fast onset and enhanced effi cacy.It is undoubtedly an eff ective adjuvant treatment method for patients and female patients with insignifi cant effi cacy or intolerance with minoxidil alone.
