多裂肌退变程度的磁共振单体素波谱评估及其与健康相关生活质量的关系

摘要 目的:探究磁共振单体素波谱序列(MRS)在中老年人群多裂肌退变定量评估中的应用价值,并分析其与健康相关生活质量的关系。方法:公开招募109例中老年受试者作为研究对象,借助MRS序列定量分析受试者多裂肌退变情况,评价指标包括:脂肪分数(FF)、水脂比(W/F)、脂肪不对称程度(FAD)。同时,所有受试者均在安静状态下填写疼痛视觉模拟评分(VAS)、功能障碍指数(ODI)、Roland-Morris腰痛功能障碍评分(RMQ)以及36项简易健康问卷评分量表(SF-36),分析两者之间的相关性。结果:多裂肌FF与VAS、ODI、RMQ呈正相关,与SF-36总分呈负相关(P<0.05);多裂肌W/F与VAS、ODI、RMQ呈负相关,与SF-36总分呈正相关(P<0.05)。结论:多裂肌FF及W/F与中老年人群健康相关生活质量密切相关,可能是反映中老年患者下腰痛、功能障碍情况及健康相关生活质量的敏感指标。 Objective:To investigate the utility of magnetic resonance spectroscopy(MRS)for quantitative assessment of lumbar multifidus muscle degeneration and its association with health-related quality of life in middle-aged and elderly individuals.Methods:This study recruited 109 middle-aged and elderly individuals and conducted an MRS-based quantitative analysis of lumbar multifidus muscle degeneration using fat fraction(FF),water-to-fat ratio(W/F),fat asymmetry degree(FAD).In addition,visual analog scale(VAS),the Oswestry Disability Index(ODI),the Roland-Morris Questionnaire(RMQ),and the 36-Item Short Form Health Survey(SF-36)were constructed to survey health status in resting state for all individuals.Correlations between lumbar multifidus muscle degeneration and health-related quality of life were analyzed.Results:The FF of lumbar multifidus muscle was positively correlated with the VAS,ODI,and RMQ scores and negatively correlated with the SF-36 score(all P<0.05).Conversely,the W/F of lumbar multifidus muscle was negatively correlated with the VAS,ODI,and RMQ scores and displayed a positive correlation with the SF-36 score(all P<0.05).Conclusion:The FF and W/F of lumbar multifidus muscle are closely associated with the health-related quality of life of middle-aged and elderly populations and thus may serve as sensitive predictors of low back pain,dysfunctional conditions,and health-related quality of life in middle-aged and elderly patients.
