补脾胃泻阴火升阳汤化裁联合胃复春片治疗慢性萎缩性胃炎伴焦虑抑郁临床观察

Efficacy observation of modified Bupiwei Xieyinhuo Shengyang decoction and Weifuchun tablets on chronic atrophic gastritis complicated with anxiety and depression

目的:观察补脾胃泻阴火升阳汤化裁治疗慢性萎缩性胃炎(CAG)伴焦虑抑郁状态的临床疗效。方法:选择符合纳入标准的慢性萎缩性胃炎伴焦虑抑郁患者80例,随机分为两组各40例,对照组予常规胃复春治疗,治疗组在对照组基础上加用补脾胃泻阴火升阳汤,治疗12周后观察两组中医证候疗效、胃黏膜病理情况、汉密尔顿焦虑量表(HAMA)评分、汉密尔顿抑郁量表(HAMD)评分以及安全性评价指标。结果:治疗12周后,治疗组中医证候疗效100%明显优于对照组的95.00%,两组比较差异有统计学意义(P﹤0.05);两组胃黏膜病理情况、HAMA及MAMD评分比较,治疗组明显优于对照组(P﹤0.05)。结论:基于李东垣“阴火理论”代表方补脾胃泻阴火升阳汤化裁联合胃复春片可有效改善慢性萎缩性胃炎患者焦虑、抑郁状态,控制不良情绪的发生,并能有效改善临床症状,降低胃黏膜征象。

Objective:To observe the clinical efficacy of modified Bupiwei Xieyinhuo Shengyang decoction on chronic atrophic gastritis(CAG)complicated with anxiety and depression.Methods:80 cases of selected patients with chronic atrophic gastritis complicated with anxiety and depression,who met inclusion criteria,were randomly divided into two groups,40 cases each group.The control group was treated with routine Weifuchun tablets,while the treatment group was treated plus with modified Bupiwei Xieyinhuo Shengyang decoction on the basis of control group.After 12 weeks of treatment,the curative efficacy of TCM syndromes,pathological conditions of gastric mucosa,Hamilton Anxiety Scale(HAMA),Hamilton Depression Scale(HAMD)and evaluation indexes of safety were observed in two groups.Results:After 12 weeks of treatment,the curative efficacy of TCM syndromes of treatment group was 100%,which was obviously better than 95.00%of control group,and the comparative difference between two groups was statistically significant(P﹤0.05);Compared the pathological condition of gastric mucosa and the scores of HAMA and MAMD between two groups,the treatment group was significantly better than control group(P﹤0.05).Conclusion:Based on Li Dongyuan's"theory of yin fire",modified Bupiwei Xieyinhuo Shengyang decoction of representative formula and Weifuchun tablets can effectively improve anxiety and depression,control the occurrence of unhealthy emotions,effectively improve clinical symptoms and reduce the symptoms of gastric mucosa.
