Expression of CD43 in adenoid cystic carcinoma and its clinical significance

目的探讨腺样囊性癌(adenoid cystic carcinoma,ACC)中CD43的表达,比较其在ACC不同组织学亚型、涎腺ACC与非涎腺ACC之间的表达差异,分析ACC中CD43与CD117表达的相关性及临床意义。方法选取2011~2019年皖南医学院弋矶山医院病理科ACC 96例和对照组103例(包括上皮-肌上皮癌18例、多形性腺瘤20例、黏液表皮样癌20例、Warthin瘤22例及基底细胞腺瘤23例),采用免疫组化EnVision两步法检测CD43和CD117蛋白表达,并分析CD43表达与ACC临床病理特征的关系。结果96例ACC中,74例CD43蛋白阳性(阳性率为77.1%),对照组CD43阳性率为3.9%;筛状型ACC 70例、管状型ACC 19例、实体型ACC 18例,其中筛状型ACC中CD43蛋白阳性57例(阳性率为81.4%),管状型ACC中CD43蛋白阳性16例(阳性率为84.2%),两者差异无统计学意义,而实体型ACC中CD43蛋白阳性仅9例(阳性率为50%),CD43蛋白阳性率较前两种分型显著降低;非涎腺ACC中CD43蛋白阳性率为22.2%(2/9),较涎腺ACC(CD43阳性率为82.8%)显著降低。96例ACC中CD117蛋白阳性86例(阳性率为89.6%),对照组阳性率为23.3%(24/103);ACC中CD43蛋白阴性患者的中位生存时间较阳性患者短,但差异无统计学意义(P>0.05)。结论CD43表达在ACC中具有一定的临床意义,且在唾液腺中优先表达,其表达随着细胞分化而降低;CD43和CD117的联合检测在ACC中具有一定的诊断意义;CD43表达可能提示着一定的预后意义。

Purpose To investigate the expression of CD43 in adenoid cystic carcinoma(ACC),and to compare the expression differences between different histological subtypes of ACC,salivary gland ACC and non-salivary gland ACC,to analyze and evaluate the expression differences of CD43 and CD117 in ACC and their clinical significance.Methods From 2011 to 2019,96 cases of ACC and 103 cases of the control group were selected from the Department of Pathology,Yijishan Hospital,Wannan Medical College,which including 18 cases of epithelial-myoepithelial carcinoma,20 cases of pleomorphic adenoma,20 cases of mucoepidermoid carcinoma,22 cases of Warthin tumor and 23 cases of basal cell adenoma.The expression of CD43 and CD117 protein were detected by immunohistochemical EnVision two-step method,and the relationship between CD43 expression and clinicopathological features of ACC was analyzed.Results In 96 cases of ACC,74 cases were positive for CD43 protein(positive rate was 77.1%),and the positive rate in the control group was 3.9%.There were 70 cases of cribriform-ACC,19 cases of tubular-ACC,and 18 cases of solid-ACC,57 cases of cribriform-ACC showed CD43 protein expression(positive rate was 81.4%),and 16 cases of tubular-ACC showed CD43 protein expression(positive rate was 84.2%),there was no significant difference between them.However,only 9 cases showed CD43 protein expression in solid-ACC(positive rate was 50%),and the positive rate of CD43 protein was significantly lower than the formers.The CD43 protein positive rate in non-salivary gland ACCs was 22.2%(2/9),which was significantly lower than that in salivary gland ACCs(82.8%).The positive rate of CD117 was 89.6%(86/96)in ACC,and 23.3%(24/103)in the control group.The median survival time of patients with negative expression of CD43 protein in ACC was shorter than that of patients with positive expression,but there was no statistical significance(P>0.05).Conclusion The expression of CD43 has certain clinical significance in ACC,and it is preferentially expressed in salivary glands,and its expression decreases with cell differentiation.The combined expression of CD43 and CD117 had certain diagnostic significance in ACC.The expression of CD43 may suggest some prognostic significance.
