超声生物显微镜、IOL Master700和Pentacam HR在ICL术前眼部生物测量中的一致性评价 被引量:2

摘要 目的评价高度近视时用超声生物显微镜(UBM)、IOL Master700和Pentacam HR测量的前房深度(ACD)值间的差异性、相关性及一致性;评价高度近视时UBM测量的水平睫状沟距离(STS)值与IOL Master700、Pentacam HR测量的水平角膜直径(WTW)值间的差异性、相关性及一致性。方法选取本院52例(104眼)高度近视人工晶状体(ICL)手术前患者,分别使用UBM、IOL Master700和Pentacam HR对双眼行生物测量并记录数值。比较三种仪器测量的ACD值和STS/WTW值,分析其相关性、一致性。以IOL Master700测量的ACD平均值3.2 mm为界线,将患者分为A组(ACD值<3.2 mm,n=56)和B组(ACD值≥3.2 mm,n=48),比较两组经三种仪器测量的STS/WTW值。结果三种仪器测量的ACD值比较,差异无统计学意义(P>0.05)。UBM测量的STS值与IOL Master700和Pentacam HR测量的WTW值三者间比较,差异具有统计学意义(P<0.05)。IOL Master700与Pentacam HR测量的ACD值比较,差异无统计学意义(P=0.38),呈高相关性(r=0.93)。UBM测量的STS值分别与IOL Master700、Pentacam HR测量的WTW值比较,差异具有统计学意义(P<0.01),且呈低相关性。UBM测量的ACD值与IOL Master700和Pentacam HR测量的ACD值差值的一致性较差;IOL Master700和Pentacam HR测量的ACD值差值的一致性好。UBM测量的STS值与IOL Master700和Pentacam HR测量的WTW值三者两两之间差值的一致性较差。B组三种仪器测量的STS/WTW值高于A组(P<0.05),其中两组UBM测量的STS值差异最大。结论IOL Master700与Pentacam HR测量的ACD值的差异性小、一致性好,必要时临床上可相互参考替代。UBM测量的STS值与IOL Master700和Pentacam HR测量的WTW值三者之间差异性较大,一致性较差,不建议相互替代。ACD≥3.2 mm时建议联合UBM测量的STS值对ICL型号进行确定。 Objective To evaluate the difference,correlation and consistency of anterior chamber depth(ACD)values measured by ultrasonic biological microscope(UBM),IOL Master700 and Pentacam HR in high myopia;to evaluate the difference,correlation and consistency of horizontal ciliary sulcus to sulcus(STS)value measured by UBM and the horizontal white to white(WTW)value measured by IOL Master700 and Pentacam HR in high myopia.Methods Fifty-two patients(104 eyes)with high myopia before implantable contact lens(ICL)surgery diagnosed in our hospital were selected.The UBM,IOL Master700 and Pentacam HR were used to perform biometrics of binoculus of all patients and the values were recorded.The ACD value and STS/WTW value measured by three instruments were compared,and their correlation and consistency were analyzed.The patients were divided into group A(ACD value<3.2 mm,n=56)and group B(ACD value≥3.2 mm,n=48)with the average ACD value of 3.2 mm measured by IOL Master700 as the boundary.The STS/WTW values measured by three instruments of the two groups were compared.Results There was no statistically significant difference in the ACD value measured by three instruments(P>0.05).The difference of the STS value measured by UBM,and the WTW value measured by IOL Master700 and Pentacam HR was statistically significant(P<0.05).There was no significant difference in the ACD value measured by IOL Master700 and Pentacam HR(P=0.38),but there was a high correlation between them(r=0.93).The STS value measured by UBM was significantly different from the WTW value measured by IOL Master700 and Pentacam HR(P<0.01),with low correlation.The differences between the ACD value measured by UBM and the ACD value measured by IOL Master700 and Pentacam HR had poor consistency;the difference of ACD value measured by IOL Master700 and Pentacam HR had good consistency.The differences between the STS value measured by UBM and the WTW value measured by IOL Master700 and Pentacam HR had poor consistency.The STS/WTW values measured by three instruments in the group B were higher than those in the group A(P<0.05),and the difference of STS value measured by UBM between the two groups was the largest.Conclusion The ACD value measured by IOL Master700 and Pentacam HR have little difference and good consistency,which can be referred to each other when necessary in clinical practice.The STS value measured by UBM and WTW value measured by IOL Master700 and Pentacam HR have great difference and poor consistency,so it is not recommended to replace each other.For ACD≥3.2 mm,it is recommended to determine the ICL model in combination with the STS value measured by UBM.
