《古今名医临证实录丛书·睡眠障碍》治疗失眠的用药规律研究

Research on the medication rules of "Gujin Mingyi Linzheng Shilu Congshu·Shuimian Zhang'ai" in the treatment of insomnia

目的采用数据挖掘方法探讨古今名医论治失眠的用药规律。方法整理《古今名医临证实录丛书·睡眠障碍》中古今名医治疗失眠的临证处方,利用中药处方建立数据库,采用古今医案云平台(V2.3)进行频次分析和聚类分析,SPSS Modeler18.0进行关联规则,SPSS Statistics25.0进行因子分析。结果共纳入44位医家,296首经验方,涉及中药331味,获得甘草、酸枣仁、茯神、茯苓、半夏等核心药物17味。药性以寒、温并用为主,药味以甘为要,归经以心经为首,功效以宁心最为多见。关联规则获得酸枣仁-茯神等核心药对18个,因子分析得到6个公因子,聚类分析得到5个聚类方。结论古今名医治疗失眠以和胃化痰、益气养血、滋阴生津、养心安神立法,常用温胆汤、五福饮、八珍汤、增液汤等方加减。

Objective To explore the medication rule of ancient and modern famous doctors in treating insomnia by data mining method.Methods The clinical prescriptions of ancient and modern famous doctors in the treatment of insomnia in"Gujin Mingyi Linzheng Shilu Congshu·Shuimian Zhang'ai"were collated,a database was established using traditional Chinese medicine formulations,the ancient and modern medical case cloud platform(V2.3)was used for frequency analysis and cluster analysis,SPSS Modeler18.0 was used for association rules,and SPSS Statistics25.0 was used for factor analysis.Results A total of 296 empirical prescriptions of 44 doctors were included,involving 331 kinds of traditional Chinese medicine,and 17 core drugs such as Gancao,Suanzaoren,Fushen,Fuling,Banxia,etc.The medicinal properties were mainly cold and warm,the medicinal taste was mainly sweet,the main meridian was the heart meridian,and the most common effect was the calming heart.According to the association rules,18 core drug pairs such as Suanzaoren-Fushen were obtained,factor analysis divides the drugs into 6 common factors,and cluster analysis divides the drugs into 5 combinations.Conclusion The core treatment methods used by ancient and modern famous doctors in the treatment of insomnia are harmonizing the stomach and resolving phlegm,supplementing Qi and nourishing blood,nourishing Yin and generating fluid,nourishing the heart and calming the mind,and commonly use modified Wendan decoction,Wufu decoction,Bazhen decoction,and Zengye decoction,etc.
