肝动脉化疗栓塞术联合CT引导下经皮微波消融术对特殊部位原发性肝癌患者的疗效分析

Effect of Hepatic Artery Chemoembolization Combined with CT-guided Percutaneous Microwave Ablation in Patients with Primary Liver Cancer at Special Sites

目的:研究肝动脉化疗栓塞术联合CT引导下经皮微波消融术对特殊部位原发性肝癌患者的疗效。方法:选取2018年1月—2020年4月蚌埠医学院第一附属医院收治的60例特殊部位原发性肝癌患者,依据抽签法将其随机分为对照组及研究组,各30例。对照组行肝动脉栓塞化疗,研究组在对照组的基础行CT引导下经皮微波消融术。观察两组的临床有效率及2年生存率、血清指标、肝功能。结果:研究组的临床有效率、2年生存率均高于对照组,差异有统计学意义(P<0.05)。治疗后,研究组的甲胎蛋白(AFP)、癌胚抗原(CEA)、血管内皮生长因子(VEGF)、谷丙转氨酶(ALT)、结合胆红素(DBIL)、总胆红素(TBIL)水平均低于对照组,差异有统计学意义(P<0.05)。结论:肝动脉化疗栓塞术联合CT引导下经皮微波消融术疗效较好,可改善特殊部位原发性肝癌患者的血清指标,促进肝功能恢复,提高其生存率。

Objective:To investigate the efficacy of hepatic artery chemoembolization combined with CT-guided percutaneous microwave ablation in patients with primary liver cancer at special sites.Method:A total of 60 primary liver cancer at special sites patients admitted to the First Affiliated Hospital of Bengbu Medical College from January 2018 to April 2020 were randomly divided into control group and research group according to the lottery method,with 30 cases in each group.The control group underwent hepatic artery chemoembolization,and the study group underwent CT-guided percutaneous microwave ablation on the basis of the control group.The clinical effective rate,2-year survival rate,serum indexes and liver function of the two groups were observed.Result:The clinical effective rate and 2-year survival rate of the study group were higher than those in the control group,the differences were statistically significant(P<0.05).After treatment,the levels of alpha-fetoprotein(AFP),carcinoembryonic antigen(CEA),vascular endothelial growth factor(VEGF),alanine aminotransferase(ALT),direct bilirubin(DBIL)and total bilirubin(TBIL)in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Hepatic arterial chemoembolization combined with CT-guided percutaneous microwave ablation has a good effect,which can improve the serum indexes of patients with primary liver cancer at special sites,promote the recovery of liver function and improve their survival rate.
