富血小板血浆技术支持下股骨颈动力交叉钉系统治疗股骨颈骨折的临床效果分析

摘要 目的:探讨富血小板血浆技术支持下股骨颈动力交叉钉系统(FNS)治疗股骨颈骨折的临床效果。方法:以2019年5月—2021年5月当阳市人民医院骨科收治70例股骨颈骨折患者为研究对象,以随机数表法分为治疗组与对照组,各35例,对照组采用小切口安装FNS内固定,治疗组采取富血小板血浆技术支持下股骨颈动力交叉钉系统治疗。对比两组治疗后骨折复位情况、疼痛视觉模拟量表(VAS)评分和Harris髋关节评分(HHS)及股骨颈短缩发生情况。结果:治疗后,两组的骨折复位质量、Garden指数、Haidukeych评分比较,差异无统计学意义(P>0.05)。术后1 d,两组VAS评分比较,差异无统计学意义(P>0.05),术后3个月、6个月及12个月,治疗组的VAS评分均低于对照组,差异有统计学意义(P<0.05)。术后6个月及12个月,治疗组HHS评分高于对照组,差异有统计学意义(P<0.05);治疗组股骨颈短缩率小于对照组,差异有统计学意义(P<0.05)。结论:富血小板血浆技术支持下股骨颈动力交叉钉系统在股骨颈骨折中的应用效果较好,且可提高髋部功能,减少术后疼痛程度及股骨颈短缩率。 Objective:To investigate the clinical effect of femoral neck dynamic cross nail system(FNS)supported by platelet rich plasma technology in the treatment of femoral neck fractures.Method:A total of 70 patients with femoral neck fracture were treated in the Orthopedic Department of Dangyang People's Hospital from May 2019 to May 2021 were selected as the study object.They were divided into treatment group and control group by random number table method,with 35 cases in each group.The control group was treated with FNS internal fixation through small incision,and the treatment group was treated with femoral neck dynamic cross nail system supported by platelet rich plasma technology.The fracture reduction,pain visual analogue scale(VAS)score and Harris hip joint score(HHS)and the occurrence of bone neck shortening were compared between the two groups after treatment.Result:After treatment,there were no statistically significant differences between the two groups in fracture reduction quality,Garden index and Haidukeych score(P>0.05).At 1 day after operation,there was no significant difference in VAS scores between the two groups(P>0.05).At 3 months,6 months and 12 months after operation,the VAS scores in the treatment group were lower than those in the control group,the differences were statistically significant(P<0.05).At 6 months and 12 months after operation,the HHS score of the treatment group were higher than those of the control group,the differences were statistically significant(P<0.05).The shortening rate of bone neck in the treatment group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:The application of femoral neck dynamic cross nail system supported by platelet rich plasma technology in the treatment of femoral neck fracture is effective,and it can improve the function of hip,reduce the degree of postoperative pain and the shortening rate of femoral neck.
