散结生津方联合低温等离子消融治疗痰浊凝滞型舌根良性增生性肿物临床研究

Clinical Study of Sanjie Shengjin Formula Combined with Low-Temperature Plasma Radiofrequency Ablation for the Treatment of Benign Hyperplastic Mass at Lingual Root Differentiated as Coagulation and Stagnation of Phlegm-Turbidity Type

【目的】探讨散结生津方联合低温等离子消融治疗痰浊凝滞型舌根部良性增生性肿物的临床疗效。【方法】将80例痰浊凝滞型舌根部良性增生性肿物患者随机分为治疗组和对照组,每组各40例。治疗组给予散结生津方联合低温等离子消融治疗,对照组单纯给予低温等离子消融治疗,疗程为2周并随访3个月。观察2组患者治疗前后的咽部异物感、吞咽阻挡感、焦虑情绪等主观症状视觉模拟量表(VAS)评分的变化情况,评价2组患者的近、远期疗效及安全性。【结果】(1)近期疗效方面:治疗2周后,治疗组的治愈率及总有效率分别为62.50%(25/40)、95.00%(38/40),对照组分别为30.00%(12/40)、72.50%(29/40);组间比较,治疗组的治愈率及总有效率均明显优于对照组(P<0.01)。(2)主观症状评分方面:治疗后,2组患者的咽部异物感、吞咽阻挡感和焦虑情绪等主观症状VAS评分均较治疗前明显降低,症状均明显改善(P<0.01),且治疗组对各项主观症状VAS评分的降低作用均明显优于对照组(P<0.05或P<0.01)。(3)远期疗效方面:治疗3个月后随访,治疗组的治愈率及总有效率分别为60.00%(24/40)、92.50%(37/40),对照组分别为27.50%(11/40)、70.00%(28/40);组间比较,治疗组的治愈率及总有效率均明显优于对照组(P<0.01)。(4)安全性方面:治疗过程中,2组患者均未出现出血等不良反应,患者的肝功能及肾功能等安全性指标也均未见异常。【结论】散结生津方联合低温等离子消融治疗痰浊凝滞型舌根部良性增生性肿物疗效确切,安全性高,可有效缓解咽部不适症状,缩短术后恢复时间,其疗效优于单纯低温等离子消融治疗。

Objective To investigate the clinical efficacy of Sanjie Shengjin Formula combined with low-temperature plasma radiofrequency ablation(LTPRA)in treating benign hyperplastic mass at lingual root differentiated as coagulation and stagnation of phlegm-turbidity type. Methods Eighty patients with benign hyperplastic mass at lingual root differentiated as coagulation and stagnation of phlegm-turbidity type were randomly divided into the treatment group and the control group,with 40 patients in each group. The treatment group was treated with the combination of Sanjie Shengjin Formula and LTPRA,while the control group was treated with LTPRA alone. Both groups were treated for 2 weeks and were followed up for 3 months. The changes in the visual analogue scale(VAS)scores of subjective symptoms such as sensation of foreign body in the pharynx, sense of obstruction during swallowing and anxiety in the two groups were observed before and after treatment. Moreover,the short-term and long-term efficacy and clinical safety of the two groups were evaluated. Results(1)After 2 weeks of treatment,the cured rate and total effective rate for the short-term efficacy in the treatment group were 62.50%(25/40)and95.00%(38/40) respectively, while those in the control group were 30.00%(12/40) and 72.50%(29/40)respectively. The intergroup comparison showed that the cured rate and total effective rate of the treatment group were significantly superior to those of the control group(P<0.01).(2)After treatment, the VAS scores of subjective symptoms such as sensation of foreign body in the pharynx,sense of obstruction during swallowing and anxiety in both groups were significantly decreased compared with those before treatment,and the symptoms were much relieved(P<0.01),and the reduction of the VAS scores in the treatment group was significantly superior to that in the control group(P<0.05 or P<0.01).(3)The follow-up after 3 months of treatment showed that the cured rate and total effective rate for the short-term efficacy in the treatment group were 60.00%(24/40)and 92.50%(37/40)respectively,and those in the control group were 27.50%(11/40)and 70.00%(28/40)respectively. The intergroup comparison showed that the cured rate and total effective rate of the treatment group were significantly superior to those of the control group(P<0.01).(4)During the treatment,no adverse reactions such as bleeding occurred in the two groups,and no abnormalities were found in the safety indicators of liver and kidney function of the patients. Conclusion The combination of Sanjie Shengjin Formula and low-temperature plasma radiofrequency ablation is effective and safe in treating benign hyperplastic mass at lingual root differentiated as coagulation and stagnation of phlegm-turbidity type. The combined therapy could effectively relieve pharyngeal discomforts and shorten the time for postoperative recovery, and its efficacy is superior to that of low-temperature plasma radiofrequency ablation alone.
