针刀“T”形术式松解治疗瘀阻脑络型无先兆偏头痛的临床观察

Clinical Observation of T-Shaped Acupotomology Surgical Release in Treating Migraine without Aura Differentiated as Blood Stasis Obstructing Brain Collateral Type

【目的】观察针刀疗法“T”形术式松解治疗瘀阻脑络型无先兆偏头痛的临床疗效。【方法】将64例瘀阻脑络型无先兆偏头痛患者随机分为观察组33例和对照组31例,对照组给予口服盐酸氟桂利嗪胶囊(西比灵)治疗,观察组给予针刀“T”形术式松解治疗,疗程为4周。观察2组患者治疗前和治疗结束后第1个月和第3个月的疼痛视觉模拟量表(VAS)评分、头痛积分量表评分及头痛影响测评量表(HIT-6)评分的变化情况,并评价2组患者的近期疗效、远期疗效和安全性。【结果】(1)治疗结束后第1个月和第3个月,观察组的总有效率分别为93.94%(31/33)和87.88%(29/33),对照组分别为87.10%(27/31)和77.42%(24/31);组间比较(秩和检验),观察组的近期疗效和远期疗效均明显优于对照组,差异均有统计学意义(P<0.05)。(2)治疗结束后第1个月和第3个月,2组患者的VAS评分、头痛积分量表和HIT-6量表的各项评分及总分均较治疗前明显降低(P<0.05),且观察组多项评分的降低幅度明显优于对照组,差异均有统计学意义(P<0.05)。(3)治疗过程中,2组患者均无不良反应和不良事件发生。【结论】针刀疗法“T”形术式松解治疗瘀阻脑络型无先兆偏头痛的临床疗效确切,可有效缓解疼痛症状及降低其对生活的影响,且近期疗效和远期疗效均优于口服西比灵。

Objective To observe the clinical efficacy of T-shaped acupotomology surgical release in treating migraine without aura differentiated as blood stasis obstructing brain collateral type. Methods Sixty-four patients with migraine without aura differentiated as blood stasis obstructing brain collateral type were randomly divided into the observation group of 33 patients and the control group of 31 patients. The control group was treated with oral use of Flunarizine Hydrochloride Capsules(Sibelium), while the observation group was treated with T-shaped acupotomology surgical release. Both groups of patients were treated for a period of 4 weeks. The changes in the visual analogue scale(VAS)scores,headache scale scores and headache impact test 6(HIT-6)scores in the two groups were observed before treatment and on the 1stand 3rdmonth of the completion of treatment. Moreover, the short-term efficacy,long-term efficacy and clinical safety of the two groups were evaluated. Results(1)On the 1st and 3rdmonth of the completion of treatment, the overall effective rate of the observation group was 93.94%(31/33)and 87.88%(29/33)respectively,and that in the control group was 87.10%(27/31)and 77.42%(24/31)respectively. The intergroup comparison tested by rank sum test showed that the short-term efficacy and long-term efficacy of the observation group were significantly superior to those of the control group,and the differences were statistically significant(P<0.05).(2)On the 1stand 3rdmonth of the completion of treatment, the VAS scores,headache scale scores and HIT-6 scores as well as the total scores in the two groups were significantly decreased compared with those before treatment(P<0.05), and the decrease in the observation group was significantly superior to that in the control group, the difference being statistically significant(P<0.05).(3)During the treatment, no adverse reactions or adverse events occurred in the two groups. Conclusion T-shaped acupotomology surgical release exerts certain effects in treating migraine without aura differentiated as blood stasis obstructing brain collateral type. The therapy could effectively relieve the pain symptoms and reduce the impact of pain on the life of the patients,and its short-term efficacy and long-term efficacy are superior to that of oral use of Flunarizine.
