胎儿胼胝体发育不全的磁共振影像学表现及其诊断价值

摘要 目的探讨胎儿胼胝体发育不全(ACC)的磁共振影像学(MRI)表现及其诊断价值。方法选择2020年4月至2022年4月在淄博市妇幼保健院接受胎儿ACC筛查的60例孕产妇作为研究对象。均行MRI检查,分析MRI影像学表现及其灵敏度、特异性。结果MRI影像学表现为三室前部和纵裂相通;侧脑室的前角外移,侧脑室的内缘出现凹陷迹象,侧脑室体被分离;侧脑室三角区域扩大;脑半球内侧脑沟呈放射状排列;侧脑室颞角长大,海马发育不全;胼胝体部分或完全发育不全,可见放射状排列脑回,表现出轻度至中度侧脑室扩张。MRI共诊断ACC 47例,其中完全型29例,部分型18例。MRI的灵敏度为98.21%,特异性为75.00%。结论MRI在ACC的诊断中可清晰地显示胎儿胼胝体情况,能够为临床诊断ACC提供参考依据。 Objective To investigate the magnetic resonance imaging(MRI)features and diagnostic value of fetal agenesis of corpus callosum(ACC).Methods A total of 60 pregnant women who received fetal ACC screening in Zibo Maternal and Child Health Care Hospital from April 2020 to April 2022 were selected as the research objects.All pregnant women underwent MRI examination,and the imaging manifestations,sensitivity and specificity of MRI were analyzed.Results MRI showed that the anterior part of the three compartments and the longitudinal fissure communicated.The anterior horn of the lateral ventricle shifted outward,the inner edge of the lateral ventricle showed signs of depression,and the body of the lateral ventricle was separated.The triangular region of the lateral compartment was enlarged.The medial sulci of the cerebral hemispheres were arranged radially.The temporal horn of the lateral ventricle was enlarged,and the hippocampus was hypoplastic.The corpus callosum was partially or completely hypoplastic with radially arranged gyri showing mild to moderate lateral ventricular dilatation.A total of 47 cases of ACC were diagnosed by MRI,including 29 cases of complete type and 18 cases of partial type.The sensitivity and specificity of MRI were 98.21%and 75.00%,respectively.Conclusion MRI can clearly display the fetal corpus callosum in the diagnosis of ACC,which can provide reference for clinical diagnosis of ACC.
