电刺激生物反馈结合盆底肌功能锻炼在产后盆底功能障碍患者中的应用效果 被引量:2

摘要 目的 探讨电刺激生物反馈结合盆底肌功能锻炼对产后盆底功能障碍患者盆底肌力、盆底功能和盆底神经肌电生理的影响。方法 选取110例产后盆底功能障碍患者作为研究对象,将其随机分为对照组和观察组,每组55例。对照组患者行盆底肌功能锻炼,观察组患者在对照组基础上行电刺激生物反馈,两组患者均干预6个月。比较两组患者干预前后盆底肌力、盆底功能、盆底神经肌电生理指标,以及干预期间功能性障碍性疾病发生情况。结果 干预前,两组患者的盆底肌力差异无统计学意义(P>0.05);干预6个月后,观察组患者盆底肌力优于对照组(P<0.05)。干预前,两组患者的盆底障碍影响简易问卷7(PFIQ-7)、盆底功能障碍问卷简表20(PFDI-20)评分差异均无统计学意义(均P>0.05);干预6个月后,两组患者的PFIQ-7、PFDI-20评分均降低,观察组患者的上述量表评分均低于对照组(均P<0.05)。干预前,两组患者的盆底肌10 s慢肌收缩平均值、60 s慢肌收缩平均值、快肌收缩最大值差异均无统计学意义(均P>0.05);干预6个月后,两组患者的盆底肌10 s慢肌收缩平均值、60 s慢肌收缩平均值、快肌收缩最大值均升高,观察组患者的上述指标均高于对照组(均P<0.05)。干预期间,观察组患者功能性障碍性疾病的发生率低于对照组(P<0.05)。结论 电刺激生物反馈结合盆底肌功能锻炼在产后盆底功能障碍患者中效果确切,能够有效地提高盆底肌力,改善盆底肌电生理指标和盆底功能。 Objective To explore the effects of electrical stimulation and biofeedback combined with pelvic floor muscle function exercise on pelvic floor muscle strength,pelvic floor function,and pelvic floor neuromuscular electrophysiology in patients with postpartum pelvic floor dysfunction.Methods A total of 110 patients with postpartum pelvic floor dysfunction were selected as research objects and were randomly divided into a control group or an observation group,with 55 patients in each group.Patients in the control group received pelvic floor muscle function exercise,based on which patients in the observation group received electrical stimulation and biofeedback additionally.Patients in both groups received an intervention for 6 months.The pelvic floor muscle strength,pelvic floor function,pelvic floor neuromuscular electrophysiological indicators before and after the intervention,as well as the occurrence of functional disorders during the intervention were compared between the two groups.Results Before the intervention,there was no statistically significant difference in pelvic floor muscle strength between the two groups(P>0.05);after 6 months of intervention,the pelvic floor muscle strength in the observation group was superior to that in the control group(P<0.05).Before the intervention,there was no statistically significant difference between the two groups in the score of Pelvic Floor Impact Questionnaire Short Form(PFIQ-7)or Pelvic Floor Distress Inventory 20(PFDI-20)(all P>0.05);after 6 months of intervention,the PFIQ-7 and PFDI-20 scores decreased in both groups,and the scores of the above-mentioned scales in the observation group were lower than those in the control group(all P<0.05).Before the intervention,there was no statistically significant difference between the two groups in the mean value of 10s slow muscle contraction,the mean value of 60s slow muscle contraction,and the maximum value of fast muscle contraction of the pelvic floor muscle(all P>0.05);after 6 months of intervention,the mean value of 10s slow muscle contraction,the mean value of 60s slow muscle contraction,and the maximum value of fast muscle contraction of the pelvic floor muscle in both groups increased,and the above-mentioned indicators in the observation group were higher than those in the control group(all P<0.05).During the intervention,the incidence of functional disorders in the observation group was lower than that in the control group(P<0.05).Conclusion Electrical stimulation and biofeedback combined with pelvic floor muscle function exercise are effective in patients with postpartum pelvic floor dysfunction,and it can effectively improve pelvic floor muscle strength,electrophysiological indicators,and pelvic floor function.
