12周太极拳联合渐进抗阻运动对膝关节骨性关节炎患者微循环功能及炎性因子的影响 被引量:10

Effects of 12 Weeks Taijiquan Combined with Progressive Resistance Exercise on Microcirculation Function and Inflammatory Factors in Patients with Knee Osteoarthritis

摘要 目的:探讨不同运动方式对膝关节骨性关节炎患者膝关节微循环功能、炎性因子及疼痛评分的影响。方法:以54名膝关节骨性关节炎患者为研究对象,分为运动A组(n=18)、运动B组(n=18)和对照组(n=18)。运动A组进行为期12周,4次/周的24式简化太极拳运动;运动B组进行12周太极拳联合渐进抗阻运动(progressive resistance exercise,PRE);对照组不进行任何形式的运动干预。实验前后对3组受试者微血管反应性、经皮氧分压、血清白介素(interleukin,IL)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、干扰素-γ(interferon-γ,IFN-γ)水平及VAS评分、Lysholm膝关节评分进行测试。结果:实验前后,3组受试者微血管反应性、IL-6、TNF-α、IFN-γ、VAS评分、Lysholm膝关节评分的组别与时间交互作用显著(P<0.05)。其中,实验后运动A组、运动B组微血管反应性、Lysholm膝关节评分显著高于实验前及对照组(P<0.05),且运动B组显著高于运动A组(P<0.05);实验后,运动A组、运动B组IL-6、TNF-α、IFN-γ、VAS评分显著低于实验前及对照组(P<0.05),且运动B组显著低于运动A组(P<0.05)。结论:12周太极拳联合PRE和太极拳运动均能显著提高膝关节骨性关节炎患者微循环功能,并降低炎性因子水平,减轻膝关节疼痛以及提高膝关节活动能力,且太极拳联合PRE干预效果更佳。 Objective:To explore the effects of different exercise modes on microcirculation function,inflammatory factors,and pain scores of knee joints in patients with knee osteoarthritis(KOA).Methods:Fifty-four patients with KOA were divided into exercise group A(n=18),exercise group B(n=18),and control group(n=18).Exercise group A received 24-style simplified Taijiquan exercises for 12 weeks,4 times a week;exercise group B received progressive resistance exercise(PRE)combined with Taijiquan exercise for 12 weeks;the control group did not receive any form of exercise intervention.Microvascular reactivity,transcutaneous partial pressure of oxygen(TcpO_(2)),interleukin(IL)level,tumor necrosis factor-α(TNF-α)level,interferon-γ(IFN-γ)level,VAS score,and Lysholm knee score were tested in the three groups before and after the test.Results:Before and after the test,the microvascular reactivity,IL-6,TNF-α,IFN-γ,VAS score,and Lysholm knee score of three groups had groups and time interaction(P<0.05).After the test,the microvascular reactivity and Lysholm knee score of exercise group A and exercise group B were significantly higher than those before the test and of the control group(P<0.05),and exercise group B were significantly higher than exercise group A(P<0.05);after the test,IL-6,TNF-α,IFN-γ,and VAS score in exercise group A and exercise group B were significantly lower than pre-test and the control group(P<0.05),and exercise group B was significantly lower than exercise group A(P<0.05).Conclusions:Both 12-week PRE combined with Taijiquan exercise and Taijiquan exercise can significantly improve the microcirculation function,reduce the level of inflammatory factors,reduce knee pain,and improve knee joint activity in patients with knee osteoarthritis,and the intervention effect of PRE combined with Taijiquan exercise is better.
