摘要
目的:探讨中性粒细胞计数与淋巴细胞计数比值(NLR)、血小板分布宽度(PDW)对早产儿颅内出血的预测价值。方法:回顾性分析121例早产儿的临床资料,根据有无颅内出血分组,比较两组间的临床资料及血液检测指标,采用ROC曲线分析NLR、PDW对新生儿颅内出血的预测价值。结果:两组在NLR、PDW、血小板计数(PLT)、淋巴细胞计数与单核细胞计数比值(LMR)方面差异有统计学意义(均P<0.05)。在性别、生产方式、母妊娠期高血压、贫血方面及其他指标差异无统计学意义(均P>0.05)。ROC曲线分析NLR联合PDW预测新生儿颅内出血的ROC曲线下面积为0.735(95%CI:0.645~0.824,P<0.01)。结论:NLR、PDW两指标联合能较好地预测早产儿ICH,有助于临床诊断。
Objective:To investigate the predictive value of neutrophil-to-lymphocyte ratio(NLR)and platelet distribution width(PDW)for intracranial hemorrhage in premature infants.Methods:The clinical data of 121 premature infants were retrospectively analyzed.The infants were divided into intracranial hemorrhage(ICH)group and non-ICH group,and their clinical data and blood test indices were compared.The predictive value of NLR and PDW for intracranial hemorrhage of neonates was analyzed by ROC curve.Results:There were significant differences in the indices of NLR,PDW,PLT,and LMR between the two groups(all P<0.05).There were no significant difference in gender,mode of delivery,maternal hypertension,anemia,and other indicators(all P>0.05).The area under ROC curve for predicting neonatal intracranial hemorrhage by NLR combined with PDW was 0.735(95%CI:0.645-0.824,P<0.01).Conclusion:The combination of NLR and PDW can better predict ICH in premature infants,and is helpful for clinical diagnosis.
