摘要
目的 探讨中性粒细胞/淋巴细胞(NLR)、血小板/淋巴细胞(PLR)与扩张型心肌病(DCM)心力衰竭的相关性。方法 选取2019年1月—2021年12月周口市中心医院诊治的扩张型心肌病患者140例为研究对象,其中58例心功能正常者,82例心力衰竭者,另选取同期80例健康体检者作为对照组,检测所有入选者的血常规,计算NLR和PLR,比较各组NLR和PLR的差异。采用logistic回归分析NLR和PLR与DCM患者发生心力衰竭的关系。采用ROC曲线分析NLR和PLR对DCM患者发生心肌衰竭的预测效能。结果 DCM心力衰竭组NLR和PLR均高于DCM心功能正常组和对照组(P<0.05);随着DCM患者心功能分级严重程度的增加,NLR和PLR水平逐渐增加(P<0.05)。logistic回归分析显示,NLR和PLR是DCM患者发生心力衰竭的独立危险因素(P<0.05)。ROC曲线分析显示,NLR和PLR两者联合对DCM发生心力衰竭的预测价值大于NLR和PLR单一指标的预测价值。结论 NLR和PLR与DCM患者心力衰竭密切相关,两者联合可有效预测DCM患者发生心力衰竭的风险。
Objective This paper aims to analyze the relationship between neutrophil/lymphocyte ratio(NLR) and platelet/lymphocyte ratio(PLR) and heart failure in patients with dilated cardiomyopathy. Methods A total of 140 DCM patients were enrolled in our study and divided into the heart failure group(n=82) and the normal cardiac function group(n=58). In addition, 80 healthy adults were selected as the control group. Blood routine was carried out in all participants and then the value of NLR and PLR were calculated. The differences of NLR and PLR among the three groups were compared. Logistic regression was used to analyze the association of heart failure in DCM with NLR and PLR. ROC curve was used to analyze and compare the predictive value of NLR and PLR for heart failure in patients with DCM. Results NLR and PLR in the heart failure group were higher than those in the normal heart function group and the control group(P<0.05). The levels of NLR and PLR increased with the increase of the severity of cardiac function classification in DCM patients(P<0.05). Logistic regression analysis showed that NLR and PLR were independent risk factors for heart failure in DCM patients(P<0.05). ROC curve analysis showed that the combined value of NLR and PLR in predicting the occurrence of heart failure in DCM was greater than that of NLR and PLR alone. Conclusion NLR and PLR are closely related to heart failure in DCM patients, and their combination can effectively predict the risk of heart failure in DCM patients.
