摘要 目的:探讨维生素D补充对维生素D缺乏的2型糖尿病患者骨代谢及糖脂代谢等指标的影响以及作用机制。方法:选择2022年1月—2022年12月住院接受治疗的80例维生素D缺乏的2型糖尿病患者作为研究对象,随机分为对照组和观察组(维生素D补充)。2组患者间性别、年龄、糖尿病病程、BMI、收缩压及舒张压无差异,具有可比性。2组患者均采用常规治疗控制、健康教育控制血糖,借助随访平台动态调整治疗方案,观察组患者在此基础上给予口服维生素D软胶囊,每日1次,每次1粒,连续观察3个月。观察治疗前后患者空腹血糖(FBG)、餐后2h血糖(PBG)、血清C肽、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、25羟维生素D[25(OH)D]、骨钙素(OC)和甲状旁腺激素(PTH)水平的变化。结果:2组患者治疗前FBG、PBG、HbAlc和血清C肽均无差异,治疗后观察组FBG、HbAlc、PBG水平均低于对照组,差异有统计学意义(P<0.05);治疗前TG、TC、LDL-C和HDL-C均无差异,治疗后观察组TG、TC水平显著降低、HDL-C水平显著升高,差异均具有统计学意义(P<0.05);治疗前25(OH)D、OC、PTH均无差异,治疗后观察组25(OH)D水平显著升高,差异具有统计学意义(P<0.05)。结论:维生素D缺乏的2型糖尿病患者使用维生素D营养素补充治疗,能够有效控制患者的糖脂代谢水平,提高患者25(OH)D水平。 Objective To explore the effect and mechanism of vitamin D supplementation on bone metabolism,glucose and lipid metabolism in type 2 diabetic patients with vitamin D deficiency.Method Totally 80 patients with type 2 diabetes with vitamin D deficiency who were hospitalized from January 2022 to December 2022 were selected as the research objects and randomly divided into the control group and the observation group(vitamin D supplementation).There is no difference in gender,age,diabetes course,BMI,systolic blood pressure and diastolic blood pressure between the two groups,which is comparable.Both groups were treated with routine treatment and health education to control bloodsugar,and the treatment plan was dynamically adjusted with the help of follow-up platform.On this basis,the patients in the observation group were given oral vitamin D soft capsules once a day,one capsule each time,for continuous observation for 3 months.Changes of the fasting blood glucose(FBG),postprandial blood glucose(PBG),serum C-peptide,triglyceride(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),25-hydroxyvitamin D[25(OH)D],osteocalcin(OC)and parathyroid hormone(PTH)level were observed.Result There was no difference in FBG,PBG,HbAlc and serum C peptide between the two groups before treatment,but the levels of FBG,HbAlc and PBG in the observation group were lower than those inthe control group after treatment,and the difference was statistically significant(P<0.05).There was no difference in TG,TC,LDL-C and HDL-C before treatment.After treatment,the levels of TG,TC and HDL-C in the observation group decreased significantly,and the differences were statistically significant(P<0.05).There was no difference in 25(OH)D,OC and PTH before treatment,but the level of 25(OH)D in the observation group increased significantly after treatment,and the difference was statistically significant(P<0.05).Conclusion Vitamin D supplementation can effectively control the level of glucose and lipid metabolism and increase the level of 25(OH)D in patients with type 2 diabetes with vitamin D deficiency.
