妊娠高血压患者住院期间患病体验与需求的Meta整合

Experience and needs of patients with Hypertensive Disorders of Pregnancy during hospitalization:a qualitative Metasynthesis

目的 :整合妊娠高血压患者患病体验,帮助临床工作者了解患者心理变化。方法 :系统检索8个中英文数据库中关于妊娠高血压患者患病体验的研究,检索时限均为建库至2022年1月,采用澳大利亚JBI循证卫生保健中心质性研究质量评价标准(2017)进行评价,采用汇集性整合方法对结果进行整合,2名研究员独立进行后交叉核对,如遇分歧与第3名研究员商讨决定。结果 :共纳入13项研究,提取51个原始主题,整合形成4个主题和8个类别。4个主题:患者缺乏疾病相关的知识,希望能够获取正确的疾病信息;疾病造成了患者生理、心理上的双重不适;患者寻求情感和医疗专业支持;患者在确诊后逐渐摆脱困扰并采取主动应对措施。结论 :妊娠高血压患者普遍缺乏疾病相关知识,渴望了解疾病的发展及自身和胎儿状况。医护人员应关注患者的心理状况,重视健康教育,动员患者家属帮助患者改善情绪,促进恢复。

Objective:To analyze the experience of patients with Hypertensive Disorders of Pregnancy(HDP).Methods:A total of eight databases(PubMed,EMBASE,Cochrane Library,Web of Science,CNKI,WanFang Data,SinoMed,VIP)were comprehensively searched for the qualitative studies from their inception to January,2022.The Joanna Briggs Institute(JBI)Critical Appraisal Checklist for Qualitative Research was used to critically assess the included studies.Qualitative data were extracted,summarized,and Meta-synthesized by two researchers independently and disagreements were discussed with a third researcher.Results:A total of 13 studies were included,51 original themes were extracted,and 4 overarching themes and 8 categories were integrated.The overarching themes were as follows:being lack of disease related knowledge and hoping to obtain appropriate disease information;double discomforts in patients'physiology and psychology;seeking emotional and medical professional support;getting rid of the trouble and taking active measures after the diagnosis.Conclusion:HDP patients generally lack disease-related knowledge,are eager to learn about the disease development,the knowledge of HDP and their own/fetal status.Nurses should pay attention to their psychological condition and provide health education,and mobilize patients'families to help them recover from the disease.
