当归补血汤对老年股骨粗隆间骨折术后髋关节功能恢复的影响

The Influence of Chinese Angelica Blood-Supplementing Decoction on the Recovery of Hip Joint Function After Surgery for Elderly Intertrochanteric Fractures of the Femur

目的:观察当归补血汤对老年股骨粗隆间骨折股骨近端防旋髓内钉(proximal femoral anti-rotation intramedullary nail, PFNA)内固定术后髋关节功能恢复的影响。方法:将86例实施PFNA内固定术的老年股骨粗隆间骨折患者按照随机数字表法分为对照组和研究组,每组各43例。对照组术后给予常规康复训练治疗,研究组在对照组治疗的基础上给予当归补血汤治疗。比较两组患者术后1个月、术后6个月、术后12个月髋关节功能恢复情况及疼痛情况,比较两组患者术后生活质量(quality of life, QOL)评分及并发症发生发生率变化情况。结果:研究组术后1个月、术后6个月、术后12个月Harris评分高于对照组,差异具有统计学意义(P<0.05)。研究组术后各项QOL评分高于对照组,差异具有统计学意义(P<0.05)。研究组在术后1个月、术后6个月、术后12个月VAS评分低于对照组,差异具有统计学意义(P<0.05)。研究组并发症发生率低于对照组,差异具有统计学意义(P<0.05)。结论:在老年股骨粗隆间骨折PFNA术后,应用当归补血汤联合康复训练治疗,可促进髋关节功能的恢复,改善患者临床症状,有效降低并发症的发生率,提高生活质量。

Objective:To observe the the influence of Chinese Angelica Blood-Supplementing Decoction on the recovery of hip joint function after proximal femoral anti rotation intramedullary nail(PFNA)internal fixation for elderly intertrochanteric fractures of the femur.Methods:A total of 86 elderly patients with femoral intertrochanteric fractures who underwent PFNA surgery were randomly divided into the control group and the research group,with 43 cases in each group.The control group received routine rehabilitation training treatment after surgery,while the research group received Chinese Angelica Blood-Supplementing Decoction on the basis of the control group's treatment.The recovery of hip joint function and pain between two groups at,as well as the changes of postoperative quality of life(QOL)scores and incidence of complications between the two groups were compared.Results:The Harris scores of the research group at the 1st month,6th month,and 12th month after surgery all were higher than those of the control group,and the differences were statistically significant(P<0.05).The postoperative QOL scores of the research group were higher than those of the control group,and the differences were statistically significant(P<0.05).The VAS scores of the research group at the 1st month,6th month,and 12th month after surgery were lower than those of the control group,and all the differences were statistically significant(P<0.05).The incidence of complications in the research group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:After PFNA surgery for elderly intertrochanteric fractures of the femur,the application of Chinese Angelica Blood-Supplementing Decoction combined with rehabilitation training can promote the recovery of hip joint function,and clinical symptoms,effectively reduce the incidence of complications,and improve quality of life.
